Indications
- Tetracycline H is the drug of choice in the following infections
- Ricketsial infection (Rocky Mountain spotted fever, endemic and scrub typhus fever and human ehrlichiosis)
- Mycoplasma pneumoniae infections in adults
- Chlamydial Infections
- Non-gonococcal or non specific urethritis
- Lyme disease
- Brucellosis
- Urinary Tract Infections with susceptible organisms
- Bronchitis in patients with known underlying chronic lung diseases
- Pelvic inflammatory disease and other sexually transmitted diseases (STDs) regimen
- Travelers diarrhea
- Acne vulgaris
- Prostatitis
- As an alternative agent in the penicillin allergic patient with syphilis
- Anaerobic infections with susceptible organisms
- Granuloma inguinale
- Cholera
- Glanders
- Relapsing fever
- V. vulnifians
Pharmacology
- Mechanism of action on protein synthesis
- Active transport system pumps the drug through the inner cytoplasmic membrane of bacteria
- Binds specifically to the 30s ribosomes
- Inhibits bacterial protein synthesis
- Susceptible Gram positive aerobic Cocci
- Resistant staphylococci, streptococci and some pneumococci
- Pseudomonas and many Enterobacteriaceae are resistant
- Active against Brucella species, Calymmatobacterium granulomatis, Vibrio cholerae and V. vulnificus
- Active against anaerobic species of bacteria
- Incompletely absorbed from the gastrointestinal tract
- Excretion in the urine and in the faeces
- Half-life of about 12 hours
Dosage & Administration
- Usual adult oral dosage of Tetracycline
- 1-2 g daily given in 2-4 divided doses
- Usual oral dosage of Tetracycline for children older than 8 years
- 25-50 mg/kg daily given in 2-4 divided doses
- Dosage for specific indications like Acne vulgaris, Acute staphylococcal infections, Acute streptococcal infections, Amoebiasis, Brucellosis, Subacute bacterial endocarditis, Syphilis
Interaction
- Impaired absorption with antacids containing divalent and trivalent cations
- Interference with the bactericidal action of penicillin
- Potentiation of the effect of anticoagulants
- Decrease in efficacy of oral contraceptives
- Possible nephrotoxic effects exacerbation by diuretics or other nephrotoxic drugs
- Increase in the hypoglycaemic effect of insulin and sulfonylureas in patients with DM
- Possible increase in toxic effects of ergot alkaloids and methotrexate
Contraindications
- Hypersensitivity to any of the tetracycline groups
- Cross-sensitivity may occur
- Systemic lupus erythematosus
- Renal impairment, particularly if severe
Side Effects
- Depression of bone growth
- Permanent gray-brown discoloration of the teeth
- Enamel hypoplasia when given during tooth development
- Hypersensitivity reactions such as anaphylaxis, urticaria and rashes
- Photosensitivity reactions
- Gastrointestinal effects like epigastric distress, nausea, vomiting
- Accentuated prerenal azotemia
- Superinfections with oral and anogenital candidiasis
- Esophageal ulcerations
Pregnancy & Lactation
- Not to be used during pregnancy
- Risk of hypertoxicity in the mother
- Effects on the developing fetus
- Potential effects in breastfeeding and in children
Precautions & Warnings
- Care in patients with impaired liver function
- Avoid high doses in such patients
- Avoid concomitant use with potential hepatotoxic drugs
