Pharmacology
- Main mechanism of action is on protein synthesis
- Active transport system pumps the drug through inner cytoplasmic membrane of bacteria
- Binds to 30s ribosomes and inhibits bacterial protein synthesis
- Susceptible to many Gram positive aerobic Cocci
- Not the drug of choice for infections due to gram positive aerobes
- Active against Brucella species, Calymmatobacterium granulomatis, Vibrio cholerae and V. vulnificus
- Active against anaerobic species of bacteria
- Incompletely absorbed from the gastrointestinal tract
- Widely distributed through body tissues and fluids
- Half-life of about 12 hours
- Excreted in the urine and in the faeces
Dosage & Administration
- Usual adult oral dosage is 1-2 g daily in 2-4 divided doses
- Usual oral dosage for children older than 8 years is 25-50 mg/kg daily in 2-4 divided doses
- Recommended to be taken one hour before or 2 hours after meals
- Specific dosages for acne vulgaris, staphyococcal infections, streptococcal infections, amoebiasis, brucellosis, subacute bacterial endocarditis, and syphilis
Interaction
- Impaired absorption with antacids containing divalent and trivalent cations, iron, zinc, and sodium bicarbonate preparations
- May interfere with the bactericidal action of penicillin
- May potentiate the effect of anticoagulants
- May decrease efficacy of oral contraceptives
- May increase the hypoglycemic effect of insulin and sulfonylureas in patients with diabetes
- May increase toxic effects of ergot alkaloids and methotrexate
Contraindications
- Contraindicated in patients hypersensitive to any of the tetracycline group
- Avoided in patients with systemic lupus erythematosus
- Considered contraindicated in severe renal impairment; if given, doses should be reduced
Side Effects
- Can cause depression of bone growth
- Can cause permanent gray-brown discoloration of the teeth
- Can cause enamel hypoplasia when given during tooth development
- Hypersensitivity reactions such as anaphylaxis, urticaria, and rashes
- Photosensitivity reactions
- Epigastric distress and nausea
- Vomiting
- Accentuated prerenal azotemia
- Superinfections with oral and anogenital candidiasis
- Esophageal ulcerations
Pregnancy & Lactation
- Not recommended during pregnancy or breastfeeding
- May result in impaired bone growth and permanent discoloration of the child's teeth if used in children up to a certain age
Precautions & Warnings
- Care should be taken if given to patients with impaired liver function
- High doses should be avoided
- Potential hepatotoxic drugs should not be given concomitantly
