Description
- A fixed-dose combination of Amlodipine and Atenolol indicated for essential hypertension, angina pectoris, post myocardial infarction, and refractory angina pectoris where nitrate therapy has failed.
Dosage and Administration
- Recommended dosage of 5/25 mg tablet once daily, may be increased to 5/25 mg two tablets daily or as advised by physicians, should be individualized.
Indications
- Essential hypertension
- Angina pectoris and hypertension as co-existing diseases
- Post myocardial infarction
- Refractory angina pectoris after failed nitrate therapy
Pharmacology
- Amlodipine - dihydropyridine calcium antagonist, inhibits transmembrane influx of calcium ions, peripheral vasodilator, reduces peripheral vascular resistance and blood pressure, decreases coronary vasoreactivity and cardiac demand
- Atenolol - cardioselective beta-blocker, reduces blood pressure by lowering cardiac output, decreasing plasma renin activity and sympathetic outflow, reduces myocardial oxygen demand
Interaction
- Disopyramide - Atenolol reduces clearance by 20%, additive negative inotropic effects on the heart may be produced
- Ampicillin - doses of 1 gm and above may reduce Atenolol levels
- Oral antidiabetics and insulin - Beta-blockers may decrease tissue sensitivity to insulin and inhibit insulin secretion
Contraindications
- Hypersensitivity to either component
- Sinus bradycardia
- Second and higher degrees of heart block
- Cardiogenic shock
- Hypotension
- Congestive heart failure
- Poor left ventricular function
Side Effects
- Fatigue
- Headache
- Edema
- Nausea
- Drowsiness
- Anxiety
- Depression
Pregnancy and Lactation
- Use during pregnancy only if expected benefit outweighs potential fetal risk
- Not recommended for nursing mothers, breastfeeding should be stopped if use is considered necessary
Precautions and Warnings
- Bronchospasm - use with caution in patients with airway obstruction
- Renal impairment - caution if creatinine clearance is less than 30 ml/min
- Hepatic impairment - caution in severe liver damage due to prolongation of Amlodipine elimination half-life
- Drug withdrawal - warn patients against sudden discontinuation, any discontinuation should be gradual and under observation
Overdose Effects
- Hypotension
- Congestive cardiac failure
- Unabsorbed drugs removal by gastric lavage or administration of activated charcoal, symptomatic treatment suggested
