Indications

  • Treatment of serious infections caused by susceptible microorganisms
  • Used for septicaemia

Pharmacology

  • Proven bactericidal antibiotic
  • Active against a broad spectrum of Gram-negative pathogens
  • Active against some Gram-positive organisms
  • Interferes with bacterial protein synthesis
  • Binds primarily to the 30S sub-units of bacterial ribosomes

Dosage & Administration

  • Given by intramuscular route
  • Usual dose for adults: 3-5 mg/kg/day in three divided doses
  • Usual dose for pediatric patients: 3 mg/kg every 12 hours for up to 2 weeks, 2 mg/kg every 8 hours for 2 weeks to 12 years
  • Dosage is the same for intravenous administration
  • For IV administration, should be diluted with sterile normal saline or 5% glucose in water
  • Infusion periods of 30 minutes to 2 hours have been advocated

Interaction

  • Concurrent use with other potentially nephrotoxic or ototoxic drugs should be avoided
  • Potent diuretics may potentiate ototoxic effects
  • Inactivated by solutions containing beta-lactam antibiotics
  • Should not be administered simultaneously with beta-lactam antibiotics

Side Effects

  • Vestibular damage
  • Reversible nephrotoxicity
  • Auditory ototoxicity
  • Symptoms include dizziness, vertigo, tinnitus, roaring in the ears, and rarely hearing loss
  • Adverse renal effects may occur

Pregnancy & Lactation

  • Safety for use in pregnancy and lactation not established
  • Crosses the placenta
  • Risk of ototoxicity in the fetus
  • Should be used only when the mother's condition justifies the risk
  • Small amounts detected in breast milk
  • Recommended to discontinue breastfeeding during therapy unless benefits outweigh risks
