Description
Belongs to the macrolides group of antibiotics. Exerts bacteriostatic or bactericidal action based on specific interference with ribosomal protein synthesis within the bacterial cell.
Pharmacology
- Mode of Action: Inhibits microsomal protein synthesis in susceptible organisms by inhibiting the translocation process. Active against many Grampositive bacteria, some Gram-negative bacteria, mycoplasmas, and chlamydia.
- Absorption: Base is destroyed by acid and is administered in the form of stable ester. Absorption rates are diminished by the presence of food.
- Blood Concentration: Peak serum concentrations are attained in 1 to 4 hours. Serum half-life is 1.2 to 4 hours.
- Distribution: Widely distributed throughout body tissue and fluids with some retention in the liver and spleen.
- Excretion: 5 to 15 % of the dose is excreted in the urine and large amounts of the unchanged active substance are excreted in the bile.
Dosage & Administration
- Adult and Children over 8 years: 250-500 mg every six hours for mild to moderate infections.
- Elderly: No special dosage recommendation.
- Children aged 2 to 8 years: 250 mg every six hours or 30-50 mg/kg body weight per day.
- Infants and Children upto 2 years: 500 mg in divided doses or 30-50 mg/kg body weight in divided doses.
Interaction
Its use in patients who are receiving high dosage of theophylline may be associated with an increase of serum theophylline levels and potential theophylline toxicity.
Contraindications
Known hypersensitivity to Erythromycin.
Side Effects
Allergic reactions are rare and mild although anaphylaxis has occurred. Occasionally there is abdominal discomfort after oral administration, sometimes with nausea and vomiting.
Pregnancy & Lactation
Caution should be exercised when prescribing this drug to pregnant patients and lactating mothers as erythromycin crosses the placental barrier and is excreted in breast milk.
Precautions & Warnings
Should be given with care in patients with impaired hepatic function, as it is excreted principally in the bile.
Overdose Effects
In case of overdosage, it should be discontinued. Overdosage should be handled with the prompt elimination of unabsorbed drug.
