Mode of Action
- Inhibits microsomal protein synthesis in susceptible organisms
- Interferes with ribosomal protein synthesis within the bacterial cell
- Bacteriostatic or bactericidal action

- Gram (+)ve Cocci and bacilli
- Gram (-)ve Neisseria catarhalis, N.meningitis, N.gonorrhoea, Haemophilus influenzae, Brodetella, H. pertussis
- Mycoplasma pneumoniae
- Legionella pneumophila
- Bacteroides fragilis, B.oralis, B.melaninogenicus
- Fusobacterium
- Chlamydia trachomatis
- Treponema pallidum
Dosage & Administration
- Adult and Children over 8 years: 250-500 mg every six hours for mild to moderate infections
- Elderly: No special dosage recommendation
- Children aged 2 to 8 years: 250 mg. every six hours or 30-50 mg/kg body weight per day
- Infants and Children upto 2 years: 500 mg. in divided doses or 30-50 mg/kg body weight in divided doses
Interaction
- Potential interaction with high dosage of theophylline leading to increased serum theophylline levels and potential theophylline toxicity
Contraindications
- Known hypersensitivity to Erythromycin

- Nausea and vomiting
Pregnancy & Lactation
- Caution advised during pregnancy and lactation due to potential placental barrier crossing and excretion in breast milk
Precautions & Warnings
- Given with care in patients with impaired hepatic function
Overdose Effects
- Discontinue in case of overdosage
- Prompt elimination of unabsorbed drug
- Not removed by peritoneal dialysis or haemodialysis
Reconstitution
- Direction for reconstitution of suspension
- Shake well before use
- Store in a cool and dry place, preferably in refrigerator
- Unused portion should be discarded after 7 days

- Keep below 25°C temperature
- Away from light & moisture
- Keep out of the reach of children
Pharmacology
- Specific binding to the 50S subunit or 70S ribosome in susceptible organisms
- Active against many Gram-positive bacteria and some Gram-negative bacteria
- Widely distributed throughout body tissue and fluids
- Enters cerebrospinal fluid when meninges are inflamed
- Crosses the placenta and is excreted in the milk
Absorption
- Erythromycin base is destroyed by acid and is therefore administered in the form of stable ester
- Diminished absorption rates in the presence of food
Blood Concentration
- Peak serum concentrations attained in 1 to 4 hours
- Serum half-life is 1.2 to 4 hours
Distribution
- Widely distributed throughout body tissue and fluids
- Some retention in the liver and spleen
- Protein binding of erythromycin base is 73%
Excretion
- 5 to 15 % of the dose of erythromycin is excreted in the urine
- Large amounts of the unchanged active substance are excreted in the bile
