Description
- Belongs to the macrolides group of antibiotics
- Exerts bacteriostatic or bactericidal action based on specific interference with ribosomal protein synthesis within the bacterial cell
- Used for the treatment of infections caused by penicillinase producing staphylococci
- Effective against a wide variety of clinical infections
Indications
- Treatment of upper respiratory tract infections like tonsillitis, sinusitis, and pharyngitis
- Effective against lower respiratory tract infections such as acute and chronic bronchitis
- Treatment of ear infections like otitis media and otitis externa
- Used for eye infections including blepharitis
- Effective against skin and soft tissue infections such as boils, impetigo, and cellulitis
- Treatment of gastrointestinal tract infections like staphylococcal enterocolitis
- Used for prophylaxis in pre and post-operative, trauma, burns, and rheumatic fever
- Effective against other infections like osteomyelitis, diphtheria, and scarlet fever
Mode of Action
- Inhibits microsomal protein synthesis in susceptible organisms
- Binds to the 50S subunit or 70S ribosome in bacteria
- Active against many Gram-positive and some Gram-negative bacteria
- Active against mycoplasmas and chlamydia
Dosage & Administration
- Adults and children over 8 years: 250-500 mg every six hours for mild to moderate infections
- Elderly: No special dosage recommendation
- Children aged 2 to 8 years: 250 mg every six hours or 30-50 mg/kg body weight per day
- Infants and children up to 2 years: 500 mg in divided doses or 30-50 mg/kg body weight in divided doses
Interaction
- Use in patients receiving high dosage of theophylline may be associated with an increase of serum theophylline levels and potential theophylline toxicity
Contraindications
- Known hypersensitivity to Erythromycin
Side Effects
- Rare allergic reactions with mild instances of anaphylaxis
- Occasional abdominal discomfort after oral administration, sometimes with nausea and vomiting
Pregnancy & Lactation
- No evidence of teratogenicity or toxicity in human studies
- Caution advised when prescribing to pregnant patients and lactating mothers
Precautions & Warnings
- Given with care in patients with impaired hepatic function as it is excreted principally in the bile
Overdose Effects
- Discontinue in case of overdosage
- Prompt elimination of unabsorbed drug and instituting appropriate measures

Reconstitution
- Shake the bottle to loosen powder
- Add boiled and cooled water to the dry powder of the bottle
- Shake well after each addition until all the powder is in suspension
- Keep the reconstituted suspension in a cool and dry place and discard the unused portion after 7 days
Storage Conditions
- Keep below 25°C, away from light and moisture
- Keep out of the reach of children
Pharmacology
- Absorption: Erythromycin base is destroyed by acid and is therefore administered in the form of stable esters
- Blood concentration: Peak serum concentrations of 0.9 to 1.4 mg/ml are attained in 1 to 4 hours after an oral dose of 500 mg
- Distribution: Widely distributed throughout body tissue and fluids with some retention in the liver and spleen
- Excretion: 5 to 15% of the dose of erythromycin is excreted in the urine and large amounts of the unchanged active substance are excreted in the bile
