Description
- A fixed-dose combination of Amlodipine, a dihydropyridine calcium antagonist, and Atenolol, a cardioselective beta-blocker.

Dosage
- Recommended dosage is one tablet daily, may be increased to two tablets daily if advised by physician.
Interaction
- Interacts with Disopyramide, Ampicillin, Oral antidiabetics and insulin.
Contraindications
- Hypersensitivity to either component, sinus bradycardia, heart block, cardiogenic shock, hypotension, heart failure, poor left ventricular function.
Side effects
- Fatigue, headache, edema, nausea, drowsiness, anxiety, depression.
Pregnancy and lactation
- Use during pregnancy only if benefit outweighs fetal risk. Not recommended for nursing mothers.
Precautions and warnings
- Use with caution in patients with airway obstruction, renal impairment, and hepatic impairment. Caution against sudden drug withdrawal.
Overdose effects
- May result in hypotension and congestive cardiac failure. Unabsorbed drugs may be removed by gastric lavage or activated charcoal.

Indications
- Essential hypertension
- Angina pectoris and hypertension as co-existing diseases
- Post myocardial infarction
- Refractory angina pectoris where nitrate therapy has failed.
Pharmacology
- Amlodipine inhibits calcium influx into vascular smooth muscle and cardiac muscle, causing vasodilation and reduced blood pressure. Atenolol lowers blood pressure by reducing cardiac output and myocardial oxygen demand.
Mode of action
- Amlodipine acts as a peripheral vasodilator, while Atenolol is a cardioselective beta-blocker.
Precaution bronchospasm
- Use with caution in patients with airway obstruction.
Precaution renal impairment
- Can be used in patients with renal impairment, caution may be necessary if creatinine clearance is less than 30 ml/min.
Precaution hepatic impairment
- Caution may be necessary in patients with severe liver damage due to prolongation of elimination half-life of Amlodipine.
Precaution drug withdrawal
- Patients should be warned against sudden drug withdrawal.

Overdose treatment
- Symptomatic treatment is suggested in cases of overdose.
Interaction disopyramide
- Atenolol reduces the clearance of disopyramide by 20%, may produce additive negative inotropic effects on the heart.
Interaction ampicillin
- Ampicillin at doses of 1 gm and above may reduce Atenolol levels.
Interaction oral antidiabetics insulin
- Beta-blockers may decrease tissue sensitivity to insulin and inhibit insulin secretion.
Contraindication hypersensitivity
- Hypersensitivity to either component.
Contraindication bradyarrhythmia
- Contraindicated in sinus bradycardia, second and higher degrees of heart block.
Contraindication cardiogenic shock
- Contraindicated in cardiogenic shock, hypotension, and heart failure.
Contraindication pregnancy
- Use during pregnancy only if the expected benefit outweighs the potential fetal risk.
Contraindication lactation
- Not recommended for nursing mothers.
