Description
- Belongs to the macrolides group of antibiotics
- Exerts a bacteriostatic or bactericidal action
- Interferes with ribosomal protein synthesis
- Effective against penicillinase producing staphylococci
- Particularly susceptible to Erymex: Gram positive cocci and bacilli, Gram negative Neisseria, Haemophilus influenzae, Mycoplasma pneumoniae, Legionella pneumophila, Bacteroides fragilis, Chlamydia trachomatis, Treponema pallidum
Pharmacology
- Inhibits microsomal protein synthesis
- Binds to 50S subunit or 70S ribosome in susceptible organisms
- Active against Gram-positive, some Gram-negative bacteria, mycoplasmas, and chlamydia
- Destroyed by acid; administered in the form of stable ester
- Widely distributed throughout body tissue and fluids
- Enters cerebrospinal fluid and crosses the placenta
- Excreted in the milk and urine
Dosage & Administration
- Adult and Children over 8 years: 250-500 mg every six hours for mild to moderate infections
- Elderly: No special dosage recommendation
- Children aged 2 to 8 years: 250 mg every six hours or 30-50 mg/kg body weight per day
- Infants and Children upto 2 years: 500 mg in divided doses or 30-50 mg/kg body weight in divided doses
Interaction
- May increase serum theophylline levels
- Associated with potential theophylline toxicity
- Theophylline dosage should be reduced during co-administration
Contraindications
- Known hypersensitivity to Erythromycin
Side Effects
- Rare allergic reactions
- Mild anaphylaxis
- Occasional abdominal discomfort after oral administration
- Nausea and vomiting
Pregnancy & Lactation
- No evidence of teratogenicity or toxicity in human studies
- Caution recommended for pregnant patients and lactating mothers
- Erythromycin crosses the placental barrier and is excreted in breast milk
Precautions & Warnings
- Should be given with care in patients with impaired hepatic function
Overdose Effects
- Discontinue Erymex in case of overdosage
- Prompt elimination of unabsorbed drug
- Other appropriate measures should be instituted

Reconstitution
- Direction for reconstitution of suspension
- Shake the bottle to loosen powder
- Add boiled and cooled water to the dry powder
- Shake well after each addition
- Reconstituted suspension should be stored in a cool and dry place, preferably in refrigerator
- Unused portion should be discarded after 7 days
