Description

  • Belongs to the macrolides group of antibiotics
  • Exerts bacteriostatic or bactericidal action based on specific interference with ribosomal protein synthesis
  • Effective against a wide variety of clinical infections
  • Particularly susceptible to Gram (+)ve : Cocci and bacilli, Gram (-)ve: Neisseria catarhalis, N.meningitis, N.gonorrhoea, Haemophilus influenzae
  • Effective against Mycoplasma pneumoniae, Legionella pneumophila, Bacteroides fragilis, B.oralis, B.melaninogenicus, Fusobacterium, Chlamydia trachomatis, Treponema pallidum

Indications

  • Treatment of upper respiratory tract infections
  • Treatment of lower respiratory tract infections
  • Treatment of ear infections
  • Treatment of eye infections
  • Treatment of skin and soft tissue infections
  • Treatment of gastrointestinal tract infections
  • Prophylaxis for pre and post-operative, trauma, burns, rheumatic fever
  • Treatment of other infections like osteomyelitis, diphtheria, scarlet fever, whooping cough

Pharmacology

  • Mechanism of action: Inhibits microsomal protein synthesis by inhibiting the translocation process
  • Activity: Active against many Gram-positive bacteria, some Gram-negative bacteria, mycoplasmas, and chlamydia
  • Absorption: Administered in the form of stable ester to avoid destruction by acid
  • Blood concentration: Peak serum concentrations attained in 1 to 4 hours after an oral dose
  • Distribution: Widely distributed throughout body tissue and fluids
  • Excretion: 5 to 15% of the dose excreted in the urine, excreted in the bile

Dosage administration

  • Adults and Children over 8 years: 250-500 mg every six hours for mild to moderate infections
  • Elderly: No special dosage recommendation
  • Children aged 2 to 8 years: 250 mg every six hours or 30-50 mg/kg body weight per day
  • Infants and Children up to 2 years: Dosage based on body weight

Interaction

May increase serum theophylline levels and potential theophylline toxicity when used in patients receiving high dosage of theophylline

Contraindications

Known hypersensitivity to Erythromycin

Side effects

  • Rare allergic reactions, mild anaphylaxis
  • Abdominal discomfort, nausea, vomiting after oral administration

Pregnancy lactation

Caution should be exercised when prescribing to pregnant patients and lactating mothers

Precautions warnings

Given with care in patients with impaired hepatic function

Overdose effects

Discontinue if overdosed; not removed by peritoneal dialysis or haemodialysis
