Description
Belongs to macrolides group of antibiotics, bacteriostatic or bactericidal action, specific interference with ribosomal protein synthesis, mainly used for penicillinase producing staphylococci, susceptible to various micro-organisms.
Pharmacology
- Mode of Action: Inhibits microsomal protein synthesis, specific binding to 50S subunit or 70S ribosome, active against many Gram-positive bacteria, some Gram-negative bacteria, mycoplasmas, and chlamydia.
- Absorption: Destroyed by acid, administered in the form of stable ester, diminished absorption rates in the presence of food.
- Blood Concentration: Peak serum concentrations, half-life, influenced by oliguria.
- Distribution: Widely distributed, some retention in liver and spleen, enters cerebrospinal fluid when meninges are inflamed, crosses placenta and excreted in milk.
- Excretion: Percentage excreted in urine and bile, unchanged active substance excreted in bile.
Dosage & Administration
- Adult and Children over 8 years: 250-500 mg every six hours for mild to moderate infections, may be increased in severe cases.
- Elderly: No special dosage recommendation, administered three times daily or twice daily by giving one-third or half of the total daily requirement 8 hourly or 12 hourly respectively.
- Children aged 2 to 8 years: 250 mg every six hours or 30-50 mg/kg body weight per day divided into four equal dosages.
- Infants and Children up to 2 years: 500 mg in divided doses or 30-50 mg/kg body weight in divided doses.
Interaction
Use in patients receiving high dosage of theophylline may be associated with an increase of serum theophylline levels and potential theophylline toxicity.
Contraindications
Known hypersensitivity to Erythromycin.
Side Effects
- Rare and mild allergic reactions
- Occasional abdominal discomfort after oral administration, sometimes with nausea and vomiting.
Pregnancy & Lactation
No evidence in humans of teratogenicity or toxicity, caution should be exercised when prescribing to pregnant patients and lactating mothers.
Precautions & Warnings
Should be given with care in patients with impaired hepatic function, as Zerobac is excreted principally in the bile.
Overdose Effects
In case of overdosage, Zerobac should be discontinued, prompt elimination of unabsorbed drug, not removed by peritoneal dialysis or haemodialysis.

Reconstitution
Shake the bottle to loosen powder, add boiled and cooled water to the dry powder, shake well after each addition until all the powder is in suspension, store in a cool and dry place, preferably in the refrigerator.

Keep below 25°C temperature, away from light and moisture, keep out of the reach of children.
