Dosage
- 500 mg once daily orally for 3 days
- 500 mg once on day 1, then 250 mg once on days 2-5 for 4 days
- 1 gm given as a single dose for sexually transmitted diseases
- 500 mg once daily for typhoid fever for 7-10 days
- 500 mg as a single daily dose by the intravenous route for at least two days for community-acquired pneumonia
- 500 mg as a single daily dose by the intravenous route for one or two days for pelvic inflammatory disease
Indications
- Lower respiratory tract infections
- Upper respiratory tract infections
- Otitis media
- Skin and soft tissue infections
- Sexually transmitted diseases including non-gonococcal urethritis and cervicitis
Mode of Action
- Binds to the 50S ribosomal subunit of susceptible microorganisms
- Interferes with microbial protein synthesis
- Actively transported to the site of infection
- Concentration in tissues can be over 50 times higher than in plasma
- High lipid solubility
Pharmacology
- Readily absorbed
- Peak concentration in adults is 2.1 to 3.2 hours for oral dosage forms
- Prolonged terminal half-life of 68 hours
- Biliary excretion is a major route of elimination
- Approximately 6% of the administered dose appears as unchanged drug in urine

- Nausea

- Allergic reactions such as rash or photosensitivity
- Reversible elevations in liver transaminases
- Rare cases of cholestatic jaundice
- Transient mild reductions in neutrophil counts

Administration
- Reconstitution procedure of suspension
- Shake the bottle well to loosen the powder
- Add boiled and cooled water up to the water mark of the bottle label
- Shake until powder is completely mixed with water
