Description
Azithrocin is indicated for infections in lower respiratory tract, upper respiratory tract, otitis media, skin and soft tissue infections, and sexually transmitted diseases. It is acid-stable, readily absorbed, and has a prolonged half-life. Azithromycin acts by binding to the 50S ribosomal subunit of susceptible microorganisms, interfering with microbial protein synthesis. It is available in oral and IV forms.
Dosage
- Adult: 500 mg once daily orally for 3 days or 500 mg once on day 1, then 250 mg once on days 2-5 for 4 days
- Children: 10 mg/kg body weight once daily for 3 days
- Azithromycin Injection: 500 mg as a single daily dose by the intravenous route for at least two days
Mode of Action
Azithromycin is actively transported to the site of infection, with high concentrations released during active phagocytosis due to ion trapping and high lipid solubility. It interferes with microbial protein synthesis by binding to the 50S ribosomal subunit.
Indications
Effective for various infections caused by susceptible organisms, including lower and upper respiratory tract infections, otitis media, skin and soft tissue infections, and sexually transmitted diseases such as non-gonococcal urethritis and cervicitis.
Administration
Azithromycin suspension can be reconstituted by shaking the powder and adding boiled and cooled water up to the water mark of the bottle label.
Interaction
- Antacid: Azithromycin should be taken at least 1 hour before or 2 hours after antacids
- Carbamazepine: No significant effect observed on plasma levels
- Cyclosporin: Caution needed before co-administration and monitoring of cyclosporin levels recommended
Contraindications
- Hypersensitivity to Azithromycin or any other macrolide antibiotic
- Co-administration of ergot derivatives
- Patients with hepatic diseases
Side Effects
- Gastrointestinal issues: nausea, abdominal discomfort, vomiting, flatulence, diarrhea
- Allergic reactions: rash, photosensitivity, rare reports of serious hypersensitivity reactions
- Elevations in liver transaminases, cases of cholestatic jaundice observed rarely
Pregnancy & Lactation
- Pregnancy Category B: No evidence of harm to the fetus in animal reproduction studies
- Caution should be exercised during pregnancy and when administered to nursing women
Precautions & Warnings
Rare serious allergic reactions such as angioneurotic edema and anaphylaxis have been reported
Use in Special Populations
- Renal impairment: No dose adjustment needed in patients with mild renal impairment
- Hepatic impairment: Not recommended for patients with hepatic disease
- No evidence of effect on ability to drive or use machines
Overdose Effects
Typical symptoms of overdosage with macrolide antibiotics include hearing loss, severe nausea, vomiting, and diarrhea. General supportive measures are indicated.
