Description

Ezith is indicated for various infections, including lower respiratory tract infections, upper respiratory tract infections, otitis media, skin and soft tissue infections, and sexually transmitted diseases caused by Chlamydia trachomatis.

Attributes

  • Dosage forms:
    • Oral
    • Injection (For IV Infusion)
  • Dosage recommendations:
    • 500 mg once daily orally for 3 days for adults
    • 500 mg once on day 1, then 250 mg once on days 2-5 for adults
    • 500 mg as a single dose orally for Chlamydia trachomatis in adults
    • 10 mg/kg body weight once daily for 3 days for children over 6 months
    • 200 mg (1 teaspoonful) for 3 days if body weight is 15-25 kg for children
    • 300 mg (1½ teaspoonfuls) for 3 days if body weight is 26-35 kg for children
    • 400 mg (2 teaspoonfuls) for 3 days if body weight is 36-45 kg for children
    • 500 mg once daily for 7-10 days in typhoid fever for children
  • Pharmacology:
    • Absorption: Greater on an empty stomach
    • Peak concentration time: 2.1 to 3.2 hours for oral dosage forms
    • Phagocytes concentration: High concentration in phagocytes
    • Tissue concentration: Over 50 times higher than in plasma
    • Half life: 68 hours
    • Biliary excretion: Major route of elimination
    • Mechanism of action: Binding to the 50S ribosomal subunit of susceptible microorganisms, interfering with microbial protein synthesis, without affecting nucleic acid synthesis
    • Activity against microorganisms:
      • Various aerobic and facultative gram-positive microorganisms
      • Aerobic and facultative gram-negative microorganisms
      • Chlamydia pneumoniae, Chlamydia trachomatis, Mycoplasma pneumoniae
      • Aerobic and facultative gram-negative microorganisms
      • Anaerobic microorganisms
  • Administration:
    • Reconstitution procedure:
      • Shake the bottle well to loosen the powder
      • Add boiled and cooled water up to the water mark of the bottle label
      • Shake until powder is completely mixed with water
    • Timing with meal: Take at least 1 hour before or 2 hours after meal
  • Interaction:
    • Antacid - should be taken at least 1 hour before or 2 hours after the antacid
    • Carbamazepine - no significant effect on the plasma levels observed
    • Cyclosporin - caution should be exercised before co-administration
    • Digoxin - possibility of raised digoxin levels should be borne in mind
    • Ergot derivatives - should not be co-administered
    • Methylprednisolone - no significant effect on the pharmacokinetics observed
    • Theophylline - no evidence of any pharmacokinetic interaction
    • Warfarin - did not alter the anticoagulant effect
    • Terfenadine - did not affect the pharmacokinetics
  • Contraindications:
    • Hypersensitivity to Azithromycin or any other macrolide antibiotic
    • Co-administration of ergot derivatives
    • Patients with hepatic diseases
  • Side effects:
    • Mild to moderate gastrointestinal side-effects
    • Allergic reactions such as rash or photosensitivity
    • Reversible elevations in liver transaminases
    • Rare cases of serious hypersensitivity reactions
    • Rare cases of cholestatic jaundice
    • Transient mild reductions in neutrophil counts
    • Reversible hearing impairment in some patients with higher doses for prolonged periods
  • Pregnancy and lactation:
    • Category: B
    • Safety in pregnant women: Should be used during pregnancy only if adequate alternatives are not available
    • Breastfeeding: Caution should be exercised when administered to nursing women
  • Precautions and warnings:
    • Rare serious allergic reactions reported
    • Caution should be exercised in using azithromycin in patients with renal impairment
  • Use in special populations:
    • Renal impairment: No dose adjustment needed in patients with mild renal impairment, caution in patients with more severe renal impairment
    • Hepatic impairment: Should not be used in patients with hepatic disease
    • Effects on ability to drive and use machines: No evidence to suggest effect on a patient’s ability
  • Overdose effects:
    • Typical symptoms:
      • Hearing loss
      • Severe nausea
      • Vomiting
      • Diarrhoea
    • Recommendations: Gastric lavage and general supportive measures
  • Storage conditions: Keep in a dry place away from light and heat, keep out of the reach of children
