Indications
- Lower respiratory tract infections treatment

Pharmacology
- Oral administration
- Rapid absorption on empty stomach
- High concentration in phagocytes
- Time to peak concentration in adults: 2.1 to 3.2 hours
- Half-life allows large single dose administration
- Biliary excretion as major elimination route
Dosage
- Adult oral dosage: 500mg once daily for 3 days
- Adult oral dosage for sexually transmitted diseases: 1gm single dose or 500mg once on day 1, followed by 250mg daily for 2 days
- Oral dosage for children based on body weight
- Azithromycin injection for IV infusion in adult patients with community-acquired pneumonia
- Oral dose administration following IV therapy
- No established safety and effectiveness of IV use in children or adolescents under 16 years
Administration
- Reconstitution procedure for suspension
- Shake bottle to loosen the powder
- Add boiled and cooled water as per label mark
- Shake until powder completely mixed with water
- Take at least 1 hour before or 2 hours after meal
Interaction
- Interaction with antacids
- Potential interaction with carbamazepine
- Caution with co-administration of cyclosporin
- Potential impairment of digoxin metabolism
- Avoid co-administration with ergot derivatives
- No significant pharmacokinetic interaction with methylprednisolone
- Co-administration with theophylline typically does not lead to pharmacokinetic interaction
- Co-administration with warfarin did not alter anticoagulant effect
- No significant effect on terfenadine pharmacokinetics
Contraindications
- Hypersensitivity to azithromycin or macrolide antibiotics
- Co-administration with ergot derivatives
- Patients with hepatic diseases
Side Effects
- Low incidence of side-effects
- Most side-effects observed were mild to moderate
- Gastrointestinal side-effects: nausea, vomiting, diarrhea, etc.
- Allergic reactions: rash, photosensitivity
- Reversible elevations in liver transaminases
- Rare cases of cholestatic jaundice
- Rare cases of transient mild reductions in neutrophil counts
- Cases of reversible hearing impairment in investigational studies
Pregnancy & Lactation
- Pregnancy Category B
- No evidence of harm to fetus in animal reproduction studies
- Caution required during pregnancy if adequate alternatives not available
- Caution for nursing women due to unknown secretion in breast milk
Precautions & Warnings
- Rare reports of serious allergic reactions
- Recurrence of symptoms in some allergic reactions
Use in Special Populations
- No dose adjustment needed in patients with mild renal impairment
- Caution in using azithromycin in patients with severe renal impairment
- Not recommended for patients with hepatic disease
- No evidence of effect on driving or using machinery
Overdose Effects
- Typical symptoms of overdosage with macrolide antibiotics
- Including hearing loss, severe nausea, vomiting, and diarrhea
- Indicated treatment: gastric lavage and general supportive measures
