Pharmacology

  • Oral administration
  • Absorption on empty stomach
  • Peak concentration in 2.1 to 3.2 hours
  • High concentration in phagocytes
  • Active transport to site of infection
  • High concentration in tissues
  • Bacterial protein synthesis inhibition
  • Active against Staphylococcus aureus
  • Active against Streptococcus pneumoniae
  • Active against Chlamydia pneumoniae
  • Active against Mycoplasma pneumoniae
  • Biliary excretion
  • Approximately 6% excretion in urine
  • Polyphasic pattern of decline in plasma concentrations
  • Mean apparent plasma clearance of 630 mL/min
  • Terminal elimination half life of 68 hours

Dosage

  • Adult oral dosage - 500 mg once daily for 3 days
  • Adult oral dosage - 500 mg on day 1, then 250 mg on days 2-5 for 4 days
  • Adult sexually transmitted diseases dosage - 1 gm single dose or 500 mg on day 1, followed by 250 mg daily for next 2 days
  • Pediatric dosage - 10 mg/kg body weight once daily for 3 days
  • Pediatric dosage - 200 mg for body weight 15-25 kg for 3 days

Administration

  • Reconstitution procedure for suspension
  • Shake the bottle well
  • Add boiled and cooled water up to water mark
  • Shake until powder is completely mixed with water
  • Take at least 1 hour before or 2 hours after a meal

  • Interaction with carbamazepine
  • Interaction with cyclosporin
  • Interaction with digoxin
  • Interaction with ergot derivatives
  • Interaction with methylprednisolone

Contraindications

  • Hypersensitivity to Azithromycin or macrolide antibiotics
  • Co-administration with ergot derivatives
  • Patients with hepatic diseases

  • Vomiting
  • Flatulence
  • Diarrhea
  • Allergic rash
  • Photosensitivity
  • Reversible elevations in liver transaminases
  • Rare cases of cholestatic jaundice
  • Mild reduction in neutrophil counts
  • Reversible hearing impairment with prolonged use

Pregnancy & Lactation

  • Pregnancy category B
  • No evidence of harm to fetus in animal studies
  • Caution in use during pregnancy if alternatives not available
  • Unknown secretion in breast milk
  • Caution in use during lactation

Precautions & Warnings

  • Rare serious allergic reactions reported
  • Caution in patients with renal impairment
  • Caution in patients with hepatic impairment

Use in Special Populations

  • No dose adjustment needed in mild renal impairment
  • Caution in more severe renal impairment
  • Not to be used in patients with hepatic disease
  • No effect on ability to drive or use machines

Overdose Effects

  • Typical symptoms of overdose with macrolide antibiotics
  • Hearing loss
  • Severe nausea, vomiting, and diarrhea
  • Gastric lavage and general supportive measures indicated

  • Keep out of the reach of children
