Pharmacology
- Absorption: Readily absorbed, greater on empty stomach
- Time to peak concentration: 2.1 to 3.2 hours for oral dosage forms
- Site of infection transport: Actively transported to the site of infection
- Half life: 68 hours
- Microbiology:
- Active against various microorganisms
- Betalactamase production has no effect on its activity
Safety warnings
- Contraindications:
- Hypersensitivity to Azithromycin or macrolide antibiotics
- Co-administration with ergot derivatives
- Patients with hepatic diseases
- Side effects:
- Gastrointestinal: nausea, abdominal discomfort, vomiting, flatulence, diarrhea
- Allergic reactions: rash, photosensitivity
- Liver transaminases elevation
- Cholestatic jaundice
- Transient mild reduction in neutrophil counts
- Reversible hearing impairment in high doses and prolonged usage
Usage guidelines
- Adult dosage oral:
- 500 mg once daily for 3 days
- 500 mg once on day 1, then 250 mg once on days 2-5 for 4 days
- Adult dosage injection:
- Community-acquired pneumonia: 500 mg as a single daily dose for at least two days by intravenous route
- Pelvic inflammatory disease: 500 mg as a single daily dose for one or two days by intravenous route
- Pediatric dosage oral:
- Child over 6 months: 10 mg/kg body weight once daily for 3 days
- Typhoid fever: 500 mg once daily for 7-10 days
Interaction with other drugs
- Antacids: Take at least 1 hour before or 2 hours after
- Carbamazepine: No significant effect observed on plasma levels
- Cyclosporin: Caution and monitoring required
- Digoxin: Possibility of raised levels should be monitored
- Ergot derivatives: Contraindicated for co-administration
- Methylprednisolone: No significant effect observed on pharmacokinetics
- Theophylline: No evidence of pharmacokinetic interaction
- Warfarin: Monitoring of prothrombin time should be continued
- Terfenadine: No significant changes in cardiac repolarization observed
Pregnancy and lactation
- Pregnancy category: Category B
- Reproduction studies: No evidence of harm to the fetus
- Breastfeeding: Caution advised
Special population use
- Renal impairment: No dose adjustment needed in mild impairment, caution in severe impairment
- Hepatic impairment: Contraindicated in patients with hepatic disease
