Indications

  • Treatment of acute bacterial exacerbation of chronic bronchitis in adults
  • Treatment of acute maxillary sinusitis
  • Treatment of community-acquired pneumonia
  • Treatment of pharyngitis or tonsillitis
  • Treatment of uncomplicated skin and skin structure infections
  • Treatment of acute otitis media in pediatric patients
  • Treatment and prophylaxis of disseminated mycobacterial infections
  • Treatment of Helicobacter pylori infection and duodenal ulcer disease in adults

Pharmacology

  • Inhibits microsomal protein synthesis in susceptible organisms
  • Binds to the donor site on the 50S subunit of the bacterial ribosome
  • Prevents translocation to the donor site
  • Active against most Gram-positive bacteria and Chlamydia
  • Active against some Gram-negative bacteria and Mycoplasmas
  • Twice the activity of Erythromycin against H. influenzae

Dosage & Administration

  • Dosage varies for different conditions and age groups
  • Combination dosing regimens for H. pylori infection
  • Specific dosages for triple and dual therapy for H. pylori infection
  • Dosage adjustment for renal and hepatic impairment

Interaction

  • Contraindicated with HMG-CoA reductase inhibitors
  • Contraindicated with gastroprokinetic agents
  • Contraindicated with ergot alkaloids
  • Contraindicated with antipsychotics
  • Contraindicated with anti-gout agents

Contraindications

  • Known hypersensitivity to clarithromycin or macrolide antibacterial drugs
  • History of cholestatic jaundice or hepatic dysfunction associated with prior use of clarithromycin

  • Allergic reactions (ranging from urticaria to anaphylaxis)

Pregnancy & Lactation

  • May be used in neonates and children in appropriate doses
  • Breast milk from mothers receiving clarithromycin should not be given to infants until treatment is completed
  • Not recommended for pregnant patients

Precautions & Warnings

  • Discontinue if severe acute hypersensitivity reactions occur
  • Avoid in patients with known QT prolongation
  • Avoid in patients receiving drugs known to prolong the QT interval
  • Discontinue if signs and symptoms of hepatitis occur
  • Increased risk of all-cause mortality in patients with coronary artery disease
  • Evaluate for Clostridium difficile associated diarrhea if diarrhea occurs
  • Exacerbation of myasthenia gravis reported in patients receiving clarithromycin therapy

Use in Special Populations

  • Safety and effectiveness established for various conditions in pediatric patients
  • Safety not established in pediatric patients under 6 months of age

Overdose Effects

  • Ingestion of large amounts can produce gastrointestinal symptoms
  • Adverse reactions should be treated by elimination of unabsorbed drug and supportive measures
  • Serum levels not expected to be affected by hemodialysis or peritoneal dialysis
