Indications

  • Acute bacterial exacerbation of chronic bronchitis in adults
  • Acute maxillary sinusitis
  • Community-acquired pneumonia
  • Pharyngitis or tonsillitis
  • Uncomplicated skin and skin structure infections
  • Acute otitis media in pediatric patients
  • Treatment and prophylaxis of disseminated mycobacterial infections
  • Helicobacter pylori infection and duodenal ulcer disease in adults

Pharmacology

  • Inhibition of microsomal protein synthesis in susceptible organisms
  • Binding to the donor site on the 50S subunit of the bacterial ribosome
  • Activity against most Gram-positive bacteria, Chlamydia, some Gram-negative bacteria, and Mycoplasmas
  • Twice the activity of Erythromycin against H. influenzae

  • Dosage for acute bacterial exacerbation of chronic bronchitis in adults
  • Dosage for acute maxillary sinusitis in adults
  • Dosage for community-acquired pneumonia in adults
  • Dosage for pharyngitis or tonsillitis in adults
  • Dosage for uncomplicated skin and skin structure infections in adults
  • Combination dosing regimens for H. pylori infection
  • Dosage for acute otitis media in pediatric patients

Interaction

  • Contraindications with HMG-CoA reductase inhibitors, gastroprokinetic agents, ergot alkaloids, antipsychotics, anti-gout agents, and antiarrhythmics drugs

Contraindications

  • Contraindicated in patients with known hypersensitivity to clarithromycin, erythromycin, or any macrolide antibacterial drugs

Pregnancy & Lactation

Precautions & Warnings

  • Discontinue if severe acute hypersensitivity reactions occur
  • Avoid in patients with known QT prolongation or receiving drugs known to prolong the QT interval
  • Discontinue if signs and symptoms of hepatitis occur
  • Increased risk of all-cause mortality in patients with coronary artery disease

Use in Special Populations

  • Safety and effectiveness in pediatric patients 6 months and older
  • Prevention of disseminated Mycobacterium avium complex (MAC) disease in pediatric patients 20 months and older with advanced HIV infection

Overdose Effects

  • Ingestion of large amounts can produce gastrointestinal symptoms
  • Altered mental status
  • Paranoid behavior
  • Hypokalemia
  • Hypoxemia
