Indications for Use

  • Acute bacterial exacerbation of chronic bronchitis in adults
  • Acute maxillary sinusitis
  • Community-acquired pneumonia
  • Pharyngitis or tonsillitis
  • Uncomplicated skin and skin structure infections
  • Acute otitis media in pediatric patients
  • Treatment and prophylaxis of disseminated mycobacterial infections
  • Helicobacter pylori infection and duodenal ulcer disease in adults

Pharmacology

  • Inhibits microsomal protein synthesis in susceptible organisms
  • Binds to the donor site on the 50S subunit of the bacterial ribosome
  • Active against most Gram-positive bacteria and Chlamydia
  • More acid-stable than Erythromycin
  • Twice the activity of Erythromycin against H. influenzae

Dosage & Administration

  • Adult dosage specified for each indication
  • Combination dosing regimens for H. pylori infection
  • Different dosages for triple therapy and dual therapy
  • Recommended daily dosage for children
  • Guidelines for use in patients with renal and hepatic impairment

Drug Interactions

  • Contraindicated with specific medications
  • Not recommended with antiarrhythmics drugs

Contraindications

  • Hypersensitivity to clarithromycin, erythromycin, or macrolide antibacterial drugs
  • History of cholestatic jaundice or hepatic dysfunction associated with prior use of clarithromycin

Pregnancy & Lactation

  • May be used in neonates and children in appropriate doses
  • Breast milk from mothers receiving Clarithromycin should not be given to infants
  • Not recommended in pregnant patients

Precautions & Warnings

  • Discontinue if severe acute hypersensitivity reactions occur
  • Avoid in patients with known QT prolongation or receiving drugs known to prolong the QT interval
  • Increased risk of all-cause mortality in patients with coronary artery disease
  • Evaluation for Clostridium difficile associated diarrhea if diarrhea occurs
  • Exacerbation of myasthenia gravis reported in patients receiving Maclar therapy

Use in Special Populations

  • Safety and effectiveness in pediatric patients aged 6 months and older
  • Not studied in MAC patients under the age of 20 months

Overdose Effects

  • Ingestion of large amounts may produce gastro-intestinal symptoms
  • Adverse reactions accompanying overdosage should be treated by the prompt elimination of unabsorbed drug
