Indications
- Treatment of acute bacterial exacerbation of chronic bronchitis in adults
- Treatment of acute maxillary sinusitis
- Treatment of community-acquired pneumonia
- Treatment of pharyngitis or tonsillitis
- Treatment of uncomplicated skin and skin structure infections
- Treatment of acute otitis media in pediatric patients
- Treatment and prophylaxis of disseminated mycobacterial infections
- Treatment of Helicobacter pylori infection and duodenal ulcer disease in adults
Pharmacology
- Inhibits microsomal protein synthesis
- Binds to the donor site on the 50S subunit of the bacterial ribosome
- Active against most Gram-positive bacteria and Chlamydia
- More acid-stable than Erythromycin
- Twice the activity of Erythromycin against H. influenzae
Dosage & Administration
- Adult dosage for acute bacterial exacerbation of chronic bronchitis: 250 mg to 500 mg every 12 hours for 7 to 14 days
- Adult dosage for acute maxillary sinusitis: 500 mg every 12 hours for 14 days
- Adult dosage for community-acquired pneumonia: 250 mg every 12 hours for 7 to 14 days
- Adult dosage for pharyngitis or tonsillitis: 250 mg every 12 hours for 10 days
- Adult dosage for uncomplicated skin and skin structure infections: 250 mg every 12 hours for 7 to 14 days
- Adult dosage for treatment and prophylaxis of disseminated Mycobacterium avium disease: 500 mg every 12 hours until the patient is considered at low risk of disseminated infection
- Recommended triple therapy dosage for H. pylori infection: Clarithromycin/lansoprazole/amoxicillin
- 15 mg/kg/day divided every 12 hours recommended for children for 10 days
Drug Interaction
- Contraindicated with HMG-CoA reductase inhibitors
- Contraindicated with gastroprokinetic agents
- Contraindicated with ergot alkaloids
- Contraindicated with antipsychotics
- Contraindicated with anti-gout agents
- Not recommended with antiarrhythmics drugs
Contraindications
- Known hypersensitivity to clarithromycin or macrolide antibacterial drugs
- History of cholestatic jaundice or hepatic dysfunction associated with prior use of clarithromycin

Pregnancy & Lactation
- Use in neonates and children in appropriate doses
- Breast milk from mothers receiving Clarithromycin should not be given to infants until treatment is completed
- Not recommended for pregnant patients
Precautions & Warnings
- Discontinue if severe acute hypersensitivity reactions occur
- Avoid in patients with known QT prolongation or receiving drugs known to prolong the QT interval
- Discontinue if signs and symptoms of hepatitis occur
Use in Special Populations
- Safety and effectiveness in pediatric patients established for various conditions
- Safety not established in pediatric patients under 6 months of age
Overdose Effects
- Ingestion of large amounts can produce gastro-intestinal symptoms
- Adverse reactions accompanying overdosage should be treated by the prompt elimination of unabsorbed drug and supportive measures
