Indications

  • Acute bacterial exacerbation of chronic bronchitis in adults
  • Acute maxillary sinusitis
  • Community-acquired pneumonia
  • Pharyngitis or tonsillitis
  • Uncomplicated skin and skin structure infections
  • Acute otitis media in pediatric patients
  • Treatment and prophylaxis of disseminated mycobacterial infections
  • Helicobacter pylori infection and duodenal ulcer disease in adults

Pharmacology

  • Inhibition of microsomal protein synthesis
  • Binding to the donor site on the 50S subunit of the bacterial ribosome
  • Preventing translocation to the donor site
  • Activity against most Gram-positive bacteria and Chlamydia
  • Acid stability
  • Twice the activity against H. influenzae compared to Erythromycin
  • Resistance of most species of Gram-negative bacteria

Dosage & Administration

  • Adult dosing for various conditions
  • Combination dosing regimens for H. pylori infection
  • Recommended pediatric dosage for different indications
  • Administration in renal and hepatic impairment

Interaction

  • Contraindicated with specific medications such as HMG-CoA reductase inhibitors and antiarrhythmics

Contraindications

  • Hypersensitivity to clarithromycin or macrolide antibacterial drugs
  • History of cholestatic jaundice or hepatic dysfunction associated with prior use of clarithromycin

  • Evaluation for Clostridium difficile associated diarrhea if diarrhea occurs
  • Exacerbation of myasthenia gravis

Use in Special Populations

  • Safety and effectiveness in pediatric patients 6 months and older for various indications
  • Use in prevention of disseminated Mycobacterium avium complex (MAC) disease in pediatric patients 20 months and older with advanced HIV infection
  • Unestablished safety and effectiveness in MAC patients under 20 months and under 6 months

Overdose Effects

  • Gastro-intestinal symptoms with large ingestion
  • Reported case of altered mental status, paranoid behaviour, hypokalemia, and hypoxemia with 8 grams ingestion
  • Treatment of adverse reactions with prompt elimination of unabsorbed drug and supportive measures
