Indications
- Treatment of serious infections caused by susceptible organisms
Pharmacology
- Exerts bacteriostatic effect on a wide range of bacteria
- Active against rickettsiae, Vibrio cholerae, Salmonella typhi, and Haemophilus influenzae
- Mode of action through interference or inhibition of protein synthesis
- Rapid absorption from GI tract
- Excretion in urine and bile
- Distribution in various body tissues
Dosage & Administration
- Prescribed at recommended doses
- Close observation for adverse reactions
- Adult dosage: 50 mg/kg/day divided into 4 doses
- Pediatric dosage: 50 mg/kg/day divided at 6 hour intervals
- Dosage adjustment for hepatic or renal impairment
- Newborn infant dosage: 25 mg/kg/day in 4 equal doses
Interaction
- Retards biotransformation of certain drugs
- Caution with concomitant use of certain antibiotics
- Competitive inhibition with erythromycin, lincomycin, and clindamycin
- Rifampin reduces Ramicol concentrations
Contraindications
- Hypersensitivity or toxic reaction to the product or its components
- Not indicated for trivial infections or viral influenza
Precautions & Warnings
- Closely monitor hematologic functions during treatment
- Cannot rely on hematologic determinations to detect bone marrow depression
Overdose Effects
- Levels exceeding 25 mcg/ml are considered toxic
- Toxicity evidenced by hemopoietic effects, increasing serum iron levels, nausea, vomiting, and diarrhea
- Charcoal hemoperfusion may be effective for removal from plasma
- Questionable value of exchange transfusion following massive overdosage, especially in neonates and infants
