Indications
- Treatment of potentially life-threatening infections
- Therapy for severe staphylococcal infections

Pharmacology
- Binds tightly to D-alanyl-D-alanine portion cell wall precursor
- Causes blockage of glycopeptide polymerisation
- Inhibits cell wall synthesis
- Causes secondary damage to the cytoplasmic membrane
Dosage & Administration
- Recommended concentration of no more than 5 mg/ml
- Recommended rate of no more than 10 mg/min in adults
- Initial dose for infants and neonates: 15 mg/kg
- Usual daily dose for adults: 2 gm
- Total daily dose for children: 40 mg/Kg
- Usual daily dosage for antibiotic-associated pseudomembranous colitis and staphylococcal enterocolitis in adults: 125 to 500 mg
- Usual daily dosage for pediatric patients: approximately 40 mg/kg
Contraindications
- Contraindicated in patients with known hypersensitivity to Vancomycin
Side Effects
- Possible anaphylactic reactions
- Rapid infusion may cause flushing of the upper body
- Rapid infusion may cause pain and muscle spasm of the chest and back
Pregnancy & Lactation
- Use only if clearly needed during pregnancy
- Excreted in human milk
- Caution should be exercised when administered to a nursing woman
Precautions & Warnings
- Patients with borderline renal function and individuals over the age of 60 should have serial tests of auditory function and of blood levels
- Periodic haematological studies, urine analysis, and renal function tests are recommended for all patients receiving the drug
- Covan is very irritating to tissue and causes injection site necrosis when injected intramuscularly
- Prolonged use may result in the overgrowth of non-susceptible organisms
- Careful observation of the patient is essential
Overdose Effects
- Supportive care is advised with maintenance of glomerular filtration
- Poorly removed from the blood by haemodialysis or peritoneal dialysis
- Haemoperfusion with Amberlite resin XAD-4 may be of limited benefit
