Indications
- Treatment of potentially life-threatening infections
- Useful in severe staphylococcal infections
- Therapy for endocarditis

Mode of Action
- Binds tightly to D-alanyl-D-alanine portion of cell wall precursor
- Blocks glycopeptide polymerisation
- Inhibits cell wall synthesis
- Causes damage to the cytoplasmic membrane
Dosage and Administration
- Concentration recommendation for adults
- Dosage for patients with normal renal function
- Dosage for patients with impaired renal function and elderly patients
- Dosage for antibiotic-associated pseudomembranous colitis
- Dosage for staphylococcal enterocolitis
- Dosage for pediatrics
Contraindications
- Not to be used in patients with known hypersensitivity to Vancomycin
Side Effects
- Well tolerated
- Possible rapid infusion-related anaphylactic reactions
- Potential flushing of upper body
- Injection site pain and thrombophlebitis
- Possible tissue irritation and injection site necrosis
- Potential overgrowth of non-susceptible organisms
Pregnancy and Lactation
- Caution in pregnancy, monitor blood levels to minimize risk of foetal toxicity
- Exercise caution when administered to nursing women
Precautions and Warnings
- Serial tests of auditory function and blood levels for patients with borderline renal function and individuals over the age of 60
- Periodic haematological studies, urine analysis, and renal function tests for all patients
- Intravenous infusion only, as injection site necrosis occurs when injected intramuscularly
- Careful observation for superinfection and pseudomembranous colitis due to C. difficile
Overdose Effects

- Maintenance of glomerular filtration
- Poor removal from blood by haemodialysis or peritoneal dialysis
- Limited benefit reported with haemoperfusion using Amberlite resin XAD-4
