Indications

  • Treatment of potentially life-threatening infections
  • Effective in severe staphylococcal infections

Pharmacology

  • Binds tightly to D-alanyl-D-alanine portion cell wall precursor
  • Blocks glycopeptide polymerisation
  • Inhibits cell wall synthesis
  • Causes damage to the cytoplasmic membrane

  • Recommended concentrations and rates for adults
  • Different dosages for patients with normal renal function and impaired renal function
  • Dosage adjustment for elderly patients
  • Capsule dosage for adults and pediatrics

Contraindications

  • Patients with known hypersensitivity to Vancomycin

  • Anaphylactic reactions
  • Hypotension
  • Wheezing
  • Dyspnoea
  • Urticaria
  • Pruritus
  • Flushing of the upper body
  • Pain and muscle spasm of the chest and back
  • Injection site necrosis when injected intramuscularly
  • Thrombophlebitis
  • Pseudomembranous colitis

Pregnancy & Lactation

  • Use in pregnancy only if clearly needed
  • Caution in nursing women
  • Unlikely significant absorption for nursing infants

Precautions & Warnings

  • Tests for auditory function in patients with borderline renal function
  • Serial tests of Vancard blood levels in patients over 60
  • Periodic haematological studies, urine analysis, and renal function tests
  • Injection site necrosis risk when injected intramuscularly
  • Superinfection risk
  • Observation for overgrowth of non-susceptible organisms
  • Reports of pseudomembranous colitis

Overdose Effects

  • Supportive care advised
  • Poor removal by haemodialysis or peritoneal dialysis
  • Limited benefit from haemoperfusion with Amberlite resin XAD-4
