Indications
- Potentially life-threatening infections

Pharmacology
- Binds tightly to D-alanyl-D-alanine portion cell wall precursor
- Blockage of glycopeptide polymerisation
- Immediate inhibition of cell wall synthesis
- Damage to the cytoplasmic membrane
Dosage & Administration
- Concentrations of no more than 5 mg/ml
- Rates of no more than 10 mg/min in adults
- Dosage adjustment in patients with impaired renal function
- Usual daily dosage for antibiotic-associated pseudomembranous colitis
- Dosage for patients with renal impairment
- Intermittent infusion is the recommended method of administration
Contraindications
- Known hypersensitivity to Vancomycin
Side Effects
- Anaphylactic reactions
- Hypotension
- Wheezing
- Dyspnoea
- Urticaria
- Pruritus
- Flushing of the upper body
- Pain and muscle spasm of the chest and back
- Infrequent if given by a slow infusion over 60 minutes
Pregnancy & Lactation
- Caution in pregnancy and lactation
- Excreted in human milk
- Unlikely to be absorbed significantly from infant's gastrointestinal tract
Precautions & Warnings
- Serial tests of auditory function in patients with borderline renal function and individuals over the age of 60
- Periodic haematological studies
- Urine analysis
- Renal function tests
- Irritating to tissue
- Injection site necrosis when injected intramuscularly
- Prolonged use may result in overgrowth of non-susceptible organisms
- Observation of the patient is essential
- Reports of pseudomembranous colitis due to C. difficile
Overdose Effects
- Supportive care advised
- Maintenance of glomerular filtration
- Poorly removed from the blood by haemodialysis or peritoneal dialysis
- Haemoperfusion with limited benefit
