Pharmacology

  • Synthetic quinolone anti-infective agent
  • Broad spectrum of activity
  • Active against gram negative aerobic bacteria
  • Active against Pseudomonas aeruginosa
  • Active against gram-positive aerobic bacteria
  • Bactericidal action via interference with DNA gyrase
  • Rapid and well absorption from GI tract
  • Widely distributed into body tissues and fluids
  • Half-life of about 3.5 hours
  • 30%-50% excretion in urine within 24 hours

Dosage

  • Dosage determined by severity and type of infection
  • Dosage determined by sensitivity of causative organism(s)
  • Dosage determined by age, weight, and renal function of patient
  • General dosage range for adults: 100-750 mg twice daily
  • Specific dosages for urinary tract infections, respiratory tract infections, gonorrhea, and other infections
  • Dosage adjustments for impaired renal function
  • No dosage adjustment for elderly patients
  • Dosage restrictions for children and adolescents
  • Duration of treatment based on severity and clinical response
  • Specific extended-release tablet dosages

Administration

  • Instruction for IV infusion administration
  • Checking the solution for leaks and integrity
  • Proper use of flow control clamp and administration set

Interaction

  • Avoid concurrent administration with certain antacids and products containing Calcium, Iron, or Zinc
  • Avoid simultaneous intake with milk or dairy products
  • Dietary calcium in a meal does not significantly affect absorption

Contraindications

  • Hypersensitivity history to Ciprofloxacin or other quinolones

  • Hypersensitivity reactions
  • Joint pain
  • Mild photosensitivity
  • Transient increase in liver enzymes
  • Risk of hypoglycemia and mental health adverse effects

Pregnancy & Lactation

  • No evidence of teratogenicity, impairment of fertility, or impaired development
  • However, shown to cause arthropathy in immature animals
  • Not recommended for use during pregnancy
  • Secreted in milk, not recommended for nursing mothers

Precautions & Warnings

  • Caution in patients with CNS disorders or factors predisposing to seizures
  • Can be taken with or without meals and with proper hydration
  • Avoid concurrent administration with certain antacids and products containing Calcium, Iron, or Zinc
  • Avoid simultaneous intake with milk or yogurt alone
  • Dietary calcium in a meal does not significantly affect absorption

Use in Special Populations

  • Effective in clinical trials but not a first choice in pediatric population

Overdose Effects

  • May lead to seizures, hallucinations, confusion, abdominal discomfort
  • May cause renal and hepatic impairment, crystalluria, and reversible renal toxicity
