Indications
- Acute bacterial exacerbation of chronic bronchitis caused by Streptococcus pneumoniae
- Community-acquired pneumonia caused by Streptococcus pneumoniae
- Haemophilus influenzae
- Mycoplasma pneumoniae
- Chlamydia pneumoniae
Pharmacology
- Bactericidal with minimum bactericidal concentrations
- Inhibition of DNA synthesis through inhibition of bacterial type II topoisomerases
- Rapid absorption after oral administration
- Widely distributed throughout the body
- Excreted in the urine and feces as unchanged drug and metabolites
Dosage & Administration
- 320 mg once daily for 5 days for acute bacterial exacerbation of chronic bronchitis
- One 320 mg tablet daily for 5 days for community-acquired pneumonia of mild to moderate severity
- One 320 mg tablet daily for 7 days for multi-drug resistant strains of Streptococcus pneumoniae, Klebsiella pneumoniae, or Moraxella catarrhalis
Interaction
- Reduced absorption when taken concomitantly with aluminium or magnesium containing antacids and iron salts
- No clinically significant interactions when co-administered with omeprazole, theophylline, digoxin, warfarin, and oral contraceptives
Contraindications
- Hypersensitivity to Gemifloxacin and other quinolones
- Previous tendon damage with fluoroquinolones
- Not for use in children under 18 years of age

Precautions & Warnings
- Dose adjustment for severe impairment of renal function
- Adequate hydration to prevent highly concentrated urine and crystalluria
- May cause dizziness; caution when operating machinery
- Tendinitis and tendon ruptures may occur, particularly in elderly patients
- Small mean increase in QTc interval observed, caution in patients predisposed to QTc interval prolongation or taking other medications that prolong the QTc interval
- Caution in patients with epilepsy
Use in Special Populations
- Dose adjustment in patients with severe renal dysfunction
- Dose modification for patients on haemodialysis or continuous ambulatory peritoneal dialysis therapy
- No dose adjustment required for hepatic impairment
- No dose adjustment required for elderly patients
Overdose Effects
- No specific antidote known
- Dialysis does not remove Gelcin sufficiently to be useful in overdose
- Symptomatic treatment and maintenance of adequate hydration recommended in the event of acute oral overdosage
