Description
Gemicin is a fluoroquinolone antibiotic indicated for the treatment of bacterial infections such as acute bacterial exacerbation of chronic bronchitis and community-acquired pneumonia. It acts by inhibiting DNA synthesis through inhibition of bacterial topoisomerases, essential for bacterial growth.
Dosage
- 320 mg once daily for 5 days for acute bacterial exacerbation of chronic bronchitis
- 320 mg once daily for 5 days for community-acquired pneumonia of mild to moderate severity
- 320 mg once daily for 7 days for multi-drug resistant infections
Pharmacology
Gemifloxacin is rapidly absorbed after oral administration and distributed throughout the body, including target tissues like the lung. It is excreted in urine and feces as unchanged drug and metabolites.
Interaction
Gemicin absorption is reduced when aluminum, magnesium-containing antacids, and iron salts are concomitantly administered. It should be taken at least 2 hours before or 3 hours after these agents. No clinically significant interactions have been observed with omeprazole, theophylline, digoxin, warfarin, and oral contraceptives.
Contraindications
Gemicin should not be used in individuals with known hypersensitivity to gemifloxacin or other quinolones, those who have previously suffered tendon damage with fluoroquinolones, and children under 18 years of age.

Pregnancy and lactation
Gemicin should not be used in pregnant or lactating women. Safety and efficacy in this population have not been established.
Precautions and warnings
- Dose adjustment to 160 mg once daily for patients with severe impairment of renal function
- Adequate hydration should be maintained to prevent highly concentrated urine and crystalluria
- Patients should avoid operating an automobile or machinery if dizziness occurs
- Tendinitis and tendon ruptures may occur, particularly in elderly patients or with co-administration of corticosteroids
- Use with caution in patients predisposed to QTc interval prolongation or taking medications known to prolong the QTc interval
- Use with caution in patients with epilepsy
Use in special populations
- Renal impairment:
- Dose adjustment not required for mild/moderate renal impairment
- For creatinine clearance <40 ml/min: 160 mg once daily
- Patients on dialysis should receive 160 mg once daily
- Hepatic impairment: Gemicin may be given to patients with hepatic impairment without dose adjustment
- Elderly patients: Dose adjustment not required
Overdose effects
No specific antidote is known. Symptomatic treatment and maintenance of adequate hydration are recommended in the event of acute oral overdosage.
