Indications
- Adjuvant treatment of postmenopausal women with hormone receptor-positive early breast cancer
- First-line treatment of postmenopausal women with hormone receptor-positive or hormone receptor unknown locally advanced or metastatic breast cancer
- Second-line treatment of advanced breast cancer in postmenopausal women with disease progression following tamoxifen therapy
- Treatment of breast cancer in patients with ER-negative disease or those who did not respond to previous tamoxifen therapy
Pharmacology
- Non-steroidal aromatase inhibitor
- Estrogen suppression in peripheral tissues
- Inhibition of the aromatase enzyme
- Action on adrenal androgens
Dosage & Administration
- One tablet of 1 mg taken once a day
- Continued use for advanced breast cancer until tumor progression
- Optimal duration of therapy for adjuvant treatment of early breast cancer unknown
- No dose changes for patients with mild-to-moderate hepatic impairment or renal impairment
- Safety and efficacy not established in pediatric patients
- Pharmacokinetics not affected by age in geriatric patients
Interaction
- Reduced plasma concentration with co-administration of tamoxifen
- Incompatibility with estrogen-containing therapies
- No significant alteration of other drugs' exposure and anticoagulant activity
- Unlikely effect on other drugs via inhibition of cytochrome P450
Contraindications
- Contraindicated in pregnancy and premenopausal women
- Hypersensitivity reactions to the drug or excipients
Side Effects
- Serious adverse reactions: skin lesions, allergic reactions, changes in liver function
- Common adverse reactions: hot flashes, asthenia, arthritis, hypertension, depression, nausea, vomiting, rash, osteoporosis, fractures, back pain, insomnia, headache, peripheral edema, cough, dyspnea, pharyngitis, and lymphedema
Pregnancy & Lactation
- Pregnancy Category X
- Unknown excretion in human milk
- Consideration for discontinuation of nursing or drug based on importance to the mother
Precautions & Warnings
- Increased incidence of ischemic cardiovascular events in patients with pre-existing ischemic heart disease
- Mean decrease in bone mineral density in lumbar spine and total hip compared to baseline
- Higher incidence of elevated serum cholesterol compared to tamoxifen users
Overdose Effects
- Well-tolerated single doses up to 60 mg in healthy male volunteers and 10 mg daily in postmenopausal women with advanced breast cancer
- No established life-threatening dose
- Lethality observed in rats at doses greater than 100 mg/kg
