Indications
- Treatment of chronic hepatitis B virus infection
- For adults and pediatric patients 2 years of age and older
- With evidence of active viral replication
- With evidence of persistent elevation in serum aminotransferases (ALT or AST) or histologically active disease
Pharmacology
- Competes with natural substrate deoxyguanosine triphosphate
- Functionally inhibits all three activities of the HBV polymerase
- Base priming
- Reverse transcription of the negative strand
- Synthesis of the positive strand of HBV DNA
- Activated by kinases
- Can be incorporated into the DNA
Dosage & Administration
- Recommended dose for nucleoside-treatment-naive adults and adolescents 16 years of age
- Recommended dose for Lamivudine-refractory patients
- Recommended dose for patients with decompensated liver disease
- To be administered on an empty stomach
- Instructions for missed dose
Interaction
- Co-administration with Lamivudine or Adefovir dipivoxil
- Effects with drugs eliminated through renal
- Effects with drugs known to affect renal function
Contraindications
- Contraindicated in patients with hypersensitivity to Entecavir
Side Effects
- Common adverse events include headache, fatigue, dizziness, and nausea
Pregnancy & Lactation
- No data on the effect of Entecavir on the transmission of HBV from mother to infant
- Unknown excretion in human milk
- Instruction not to breastfeed while taking Entecavir
Precautions & Warnings
- Lactic acidosis
- Exacerbations of hepatitis after discontinuation of treatment
Use in Special Populations
- Safety and effectiveness in pediatric patients below the age of 2 years not established
- Care in dose selection for geriatric patients
- Dose adjustment in renal impairment based on creatinine clearance
