Indications
- Treatment of HIV-1 infection
- Treatment of chronic hepatitis B in adults
Pharmacology
- Mechanism of action: Inhibition of HIV reverse transcriptase
- Pharmacokinetics: Water-soluble diester prodrug of Tenofovir
- Oral bioavailability of approximately 25%
- Maximum serum concentrations achieved in 1.0-10.4 hrs
- Low protein binding to plasma and serum proteins
- Half-life of approximately 17 hours
- Elimination by glomerular filtration and active tubular secretion
Dosage & Administration
- Size and frequency of dosage must be adjusted to individual patient
- Adult dose: 245 mg Tenofovir tablet once daily, orally, regardless of food
- Pediatric dose (12 years and older, ≥35 kg): 245 mg Tenofovir tablet once daily, orally, regardless of food
Interaction
- Coadministration caution with didanosine and close monitoring for adverse reactions
- Monitoring for adverse reactions when used with atazanavir, lopinavir, ritonavir
Contraindications
- Broken bone due to disease or illness
- Enlarged fatty liver
- Severe liver disease
- Acute kidney disease
- Decreased bone calcification or density
- Breastfeeding mother
- Low phosphate levels in blood
- Increased blood acidity due to high lactic acid levels
- Overweight
Side Effects
- Common: Headache, diarrhea, nausea, weakness
- Other possible: Indigestion, heartburn, chest pain, drowsiness
- Report immediately: High cholesterol, kidney problems, depression, anxiety
Pregnancy & Lactation
- Pregnancy category B
- Use during pregnancy only if clearly needed
- CDC recommendation against breastfeeding by HIV-1-infected mothers
Precautions & Warnings
- Lactic acidosis and severe hepatomegaly with steatosis reported
- Exacerbation of hepatitis after treatment discontinuation
- New onset or worsening renal impairment possible
- Avoid use with certain combination products and adefovir disoproxil
- Use caution in HIV-1 and HBV coinfected patients
- Potential for fat redistribution, immune reconstitution syndrome
- Dose adjustment for renal impairment
Use in Special Populations
- Dose adjustment for renal impairment in adults
- Increased drug exposures in moderate to severe renal impairment
- No adjustment necessary for mild renal impairment
