Indications
- Treatment of HIV-1 infection
- Treatment of chronic hepatitis B in adults
Mechanism of Action
- Inhibition of HIV reverse transcriptase
- Competition with natural substrate deoxyadenosine 5-triphosphate
Pharmacokinetics
- Water soluble diester prodrug
- Oral bioavailability of approximately 25%
- Maximum serum concentrations achieved in 1.0-10.4 hrs
- Protein binding less than 0.7% to human plasma proteins and less than 7.2% to serum proteins
- Half life of approximately 17 hours
- Elimination by glomerular filtration and active tubular secretion
Dosage & Administration
- Oral administration once daily without regard to food
- Adult dose: 245 mg tablet once daily
- Pediatric dose: 245 mg tablet once daily for those 12 years and older and over 35 kg
Interactions
- Cautious coadministration with didanosine and close monitoring for adverse reactions
- Monitoring for T-Fovir-associated adverse reactions when coadministered with atazanavir or lopinavir/ritonavir
Contraindications
- Broken bone due to disease or illness
- Enlarged fatty liver
- Severe liver disease
- Acute kidney disease
- Decreased calcification or density of bone
- Breastfeeding
- Low amount of phosphate in the blood
- Increased blood acidity due to high levels of lactic acid
- Overweight

Pregnancy & Lactation
- Pregnancy category B
- Use during pregnancy only if clearly needed
- Avoid breastfeeding to prevent postnatal transmission of HIV-1
- No adequate and well-controlled studies in pregnant women
Precautions & Warnings
- Lactic acidosis/severe hepatomegaly with steatosis
- Exacerbation of hepatitis after discontinuation of treatment
- New onset or worsening renal impairment
- Coadministration with other products containing T-Fovir
- Use caution in patients coinfected with HIV-1 and HBV
- Fat redistribution in HIV-infected patients
- Immune reconstitution/redistribution syndrome
Use in Special Populations
- Dose adjustment for renal impairment in adults
- Increased drug exposures in patients with moderate to severe renal impairment
- No dose adjustment necessary for patients with mild renal impairment
