Indications
- Treatment of HIV-1 infection
- Treatment of chronic hepatitis B in adults
Pharmacology
- Mechanism of Action: Inhibition of HIV reverse transcriptase
- Pharmacokinetics: Water soluble diester prodrug of Tenofovir, low protein binding, half life of approximately 17 hours
Dosage & Administration
- Size and frequency of dosage adjusted to individual patient
- Oral administration once daily, without regard to food
- Recommended dose of 245 mg tablet for adults
- Recommended dose for pediatric patients 12 years of age and older with body weight greater than or equal to 35 kg
Interaction
- Coadministration with didanosine should be cautious
- Monitoring required when coadministered with atazanavir or lopinavir/ritonavir
Contraindications
- Broken bone, enlarged fatty liver, severe liver disease, acute kidney disease, decreased bone calcification or density, breastfeeding, low blood phosphate, high blood acidity, overweight
Side Effects
- Common: Headache, diarrhea, nausea, weakness
- Other possible side effects include indigestion, heartburn, chest pain, drowsiness
- Serious side effects: High cholesterol, kidney problems, depression, anxiety
Pregnancy & Lactation
- Pregnancy category B
- No adequate and well-controlled studies in pregnant women
- Use during pregnancy only if clearly needed
- Avoid breastfeeding to prevent postnatal transmission of HIV-1
- Limited data on pediatric and geriatric use
Precautions & Warnings
- Lactic acidosis/severe hepatomegaly with steatosis reported
- Exacerbation of hepatitis after discontinuation of treatment
- Renal impairment and acute renal failure reported
- Avoid coadministration with specific products
- Risk of HIV-1 resistance in coinfected patients
- Fat redistribution and immune reconstitution syndrome observed
Use in Special Populations
- Dose adjustment for patients with renal impairment based on creatinine clearance
