  • Tinea unguium (onychomycosis; ringworm of the nails)
  • Tinea cruris (ringworm of the thigh)
  • Tinea barbae (barber's itch)

Pharmacology

Fungistatic, inhibits fungal cell mitosis and nuclear acid synthesis, interferes with spindle and cytoplasmic microtubules by binding to alpha and beta tubulin, binds to keratin in human cells and fungal microtubes altering the mitosis

Dosage

  • Accurate diagnosis: Essential, by direct microscopic examination or culture on appropriate medium
  • Medication duration:
    • Tinea capitis: 4 to 6 weeks
    • Tinea corporis: 2 to 4 weeks
    • Tinea pedis: 4 to 8 weeks
    • Tinea unguium fingernails: at least 4 months
    • Tinea unguium toenails: at least 6 months
  • General measures: Hygiene control, concomitant use of topical agents may be required

Administration

Should be taken with food, immediately after meals

Interaction

  • Warfarin interaction: May require dosage adjustment
  • Barbiturates interaction: May depress the activity of G-G Vin
  • Oral contraceptives interaction: May reduce efficacy and increase the incidence of breakthrough bleeding

Contraindications

  • Contraindicated in:
    • patients with porphyria
    • hepatocellular failure
    • individuals with a history of hypersensitivity to griseofulvin
  • Pregnancy contraindication: Two cases of conjoined twins reported, should not be prescribed to pregnant patients

Side Effects

  • Hypersensitivity reactions:
    • skin rashes
    • urticaria
    • angioneurotic edema
    • erythema multiforme-like drug reaction
  • Other side effects:
    • Paresthesias of the hands and feet
    • oral thrush
    • nausea
    • vomiting
    • epigastric distress
    • diarrhea
    • headache
    • fatigue
    • dizziness
    • insomnia
    • mental confusion
    • impairment of routine activities
    • Proteinuria
    • Leukopenia

Pregnancy and Lactation

Pregnancy Category C, potential benefit justifying the potential risk to the fetus

Precautions and Warnings

  • Close observation: Patients on prolonged therapy with any potent medication should be under close observation, periodic monitoring of organ system function should be done
  • Cross sensitivity: Cross sensitivity with penicillin exists
  • Photosensitivity reaction: Patients should be warned to avoid exposure to intense natural or artificial sunlight
