Description

• Aprepitant medication for prevention of postoperative nausea and vomiting (PONV) and chemotherapy induced nausea and vomiting (CINV)

Indications

• Prevention of postoperative nausea and vomiting (PONV)
• Prevention of Chemotherapy Induced Nausea and Vomiting (CINV)

Pharmacology

• Selective high affinity antagonist of human substance P neurokinin 1 (NK1) receptors

Dosage

• Recommended oral dosage of 40 mg within 3 hours prior to induction of anesthesia for post operative nausea and vomiting
• Specific regimen for prevention of nausea and vomiting associated with highly emetogenic and moderately emetogenic cancer chemotherapy
• Administration without dosage adjustment for elderly patients

Administration

• Can be taken with or without food

Interaction

• Substrate, weak-to-moderate inhibitor, and inducer of CYP3A4 and CYP2C9
• Precautions for coadministration with drugs using CYP3A4 or CYP2C9
• Reduction in efficacy of hormonal contraceptives
• Need for alternative or back-up methods of contraception during treatment and for 1 month following the last dose

Contraindications

• Hypersensitivity to any component of the product
• Concurrent use with Pimozide, Terfenadine, Astemizole, Cisapride

Pregnancy & Lactation

• Pregnancy Category B
• Use only if clearly needed during pregnancy
• Unknown excretion in human milk
• Importance-based decision for nursing continuation

Use in Special Populations

• No dosage adjustment for patients with renal impairment or end-stage renal disease (ESRD) undergoing hemodialysis
• No dosage adjustment for patients with mild to moderate hepatic impairment
• Lack of clinical data in patients with severe hepatic impairment

Overdose Effects

• No specific information available on treatment of overdosage
• Generally well tolerated up to single doses of 600 mg
• Possible drowsiness and headache
• Discontinuation in case of overdose
• General supportive treatment and monitoring
• Ineffectiveness of medicine-induced emesis
• Incapability of removal by hemodialysis
