Indications
- Vaginal Candidiasis

Pharmacology
- Triazole antifungal agent
- Inhibitor of fungal cytochrome P-450 dependent enzymes
- Responsible for the synthesis of ergosterol in fungal cell membrane
Dosage & Administration
- Single dose of 150 mg for Vaginal Candidiasis
- 200 mg in 1st day followed by 100 mg daily for 14 days for Oropharyngeal Candidiasis
- 200 mg in 1st day followed by 100 mg daily for 14-30 days for Oesophageal Candidiasis
- 150 mg weekly for 4-6 weeks for Tinea corporis/Tinea cruris/Tinea pedis/Other Tinea
- 50 mg daily for 20 days for Kerion
- 400 mg as a single dose for Pityriasis versicolor
- 150 mg weekly for 12 months for Onychomycosis
- 400 mg on first day then 200-400 mg daily for Invasive candidal infections and cryptococcal infections (including meningitis)
- 200 mg daily for Prevention of cryptococcal meningitis
- 50-400 mg daily for Prevention of fungal infections in immunocompromised patients
- 1-2 mg/kg daily for Superficial Candidiasis
- 3-6 mg/kg daily for Systemic Candidiasis & Cryptococcal infection
- Dosage interval modifications for Renal Impairment
- Doses up to 12 mg/kg in children aged 5-13 years
Interaction
- Increased prothrombin time with warfarin
- Prolonged serum half-life of oral sulphonylureas
- Increased plasma concentrations with hydrochlorothiazide
- Increased level of phenytoin
- Decreased AUC and shorter half-life with rifampicin
- Increased AUCs of ethinyl estradiol and levonorgestrel with combined oral contraceptive
- Slowly increased cyclosporin concentrations
- Possible impairment of absorption with food, cimetidine, antacids, or following total body irradiation
- Decreased mean plasma clearance of theophylline
Contraindications
- Hypersensitivity to Fluconazole or related triazole compounds
Side Effects
- Nausea
- Abdominal discomfort
- Diarrhoea
- Flatulence
- Rarely encountered rash
- Rare cases of anaphylaxis
Pregnancy & Lactation
- Adverse fetal effects in animals
- Little use during human pregnancy
- Not recommended during pregnancy or in women of childbearing potential unless adequate contraception is employed
- Found in human breast milk at concentrations similar to plasma, hence not recommended during lactation
Precautions & Warnings
- Abnormalities of hepatic, renal, haematological, and other biochemical function tests observed in some patients
- Rare cases of hepatic necrosis
- Rare development of exfoliative cutaneous reactions
- Discontinuation of therapy if rash develops
- Not recommended for use during lactation
- Unlikely to impair the ability to drive or use machinery
Use in Special Populations
- Normal dose usage recommended for the elderly if no evidence of renal impairment
- Dosage interval adjustments for Renal Impairment
- Excreted predominantly in the urine as unchanged drug
- Use with caution in patients with serious underlying diseases
Overdose Effects
- Supportive and symptomatic treatment
- Possible adequacy of gastric lavage if necessary
- Excretion in urine, forced volume diuresis may increase elimination rate
- Haemodialysis decreases plasma levels by approximately 50%
