Indications

Pharmacology

Potent inhibitor of fungal cytochrome P-450 dependent enzymes

Dosage & Administration

  • Adults:
    • 150 mg as a single dose for Vaginal Candidiasis
    • 200 mg in 1st day followed by 100 mg daily for 14 days for Oropharyngeal Candidiasis
    • 200 mg in 1st day followed by 100 mg daily for 14-30 days for Oesophageal Candidiasis
    • 150 mg weekly for 4-6 weeks for Tinea infections
    • 50 mg daily for 20 days for Kerion
    • 400 mg as a single dose for Pityriasis versicolor
    • 150 mg weekly for 12 months for Onychomycosis
    • Orally or by IV infusion, 400 mg on first day then 200-400 mg daily for invasive infections
    • Others
  • Use in Specific Population:
    • Dose adjustments in elderly
    • Dose adjustments in renal impairment

Interaction

  • Warfarin: Increased prothrombin time
  • Sulphonyl ureas: Prolonged serum half-life
  • Hydrochlorothiazide: Increased plasma concentrations
  • Phenytoin: Increased phenytoin levels
  • Rifampicin: Decreased AUC and shorter half-life
  • Oral contraceptives: No relevant effects

Contraindications

Known hypersensitivity to Fluconazole or related triazole compounds

  • Flatulence
  • Rash
  • Anaphylaxis (rare)

Pregnancy & Lactation

Adverse fetal effects seen in animals at high dose levels

Precautions & Warnings

  • Abnormalities in hepatic, renal, haematological tests
  • Hepatic necrosis in severe underlying disease patients
  • Exfoliative cutaneous reactions (rare)
  • Discontinue if rash develops
  • Avoid use in lactation
  • Unlikely to impair ability to drive or use machinery

Use in Special Populations

  • Elderly patients: Use normal dose unless evidence of renal impairment
  • Renal impairment: Dose adjustments based on creatinine clearance

Overdose Effects

Supportive and symptomatic treatment, gastric lavage if necessary, haemodialysis may decrease plasma levels by 50%
