Pharmacology

Triazole Antifungal Agent - potent inhibitor of fungal cytochrome P-450 dependent enzymes.

Dosage & Administration

  • Adults:
    • 150 mg as a single dose for Vaginal Candidiasis
    • 200 mg in 1st day followed by 100 mg daily for 14 days for Oropharyngeal Candidiasis
    • 200 mg in 1st day followed by 100 mg daily for 14-30 days for Oesophageal Candidiasis
    • 150 mg weekly for 4-6 weeks for Tinea Corporis/Cruris/Pedis/Other Tinea
    • 50 mg daily for 20 days for Kerion
    • 400 mg as a single dose for Pityriasis Versicolor
    • 150 mg weekly for 12 months for Onychomycosis
    • Orally or by IV infusion, 400 mg on first day then 200-400 mg daily for Invasive Candidal Infections and Cryptococcal Infections
    • Orally or by IV infusion 200 mg daily for Prevention of Cryptococcal Meningitis
    • Orally or by IV infusion, 50-400 mg daily for Prevention of Fungal Infections in Immunocompromised Patients
  • Children:
    • Over 1 year:
      • 1-2 mg/kg daily for Superficial Candidiasis
      • 3-6 mg/kg daily for Systemic Candidiasis & Cryptococcal Infection
  • Use in Specific Population:
    • Elderly patient: Use normal dose if no evidence of renal impairment
    • Renal Impairment: No adjustment in single dose therapy is required. Multiple dose therapy should be modified based on creatinine clearance.

Interaction

  • Increased prothrombin time after warfarin administration
  • Prolonged serum half-life of concomitantly administered oral sulphonylureas
  • Increased plasma concentrations of fluconazole with multiple-dose hydrochlorothiazide
  • Potential increase in the level of phenytoin
  • Decreased AUC and shorter half-life of fluconazole with rifampicin
  • No relevant effects on hormone levels in combined oral contraceptive study
  • Slow increase in cyclosporin concentrations
  • No clinically significant impairment of fluconazole absorption with food, cimetidine, antacids, or following total body irradiation
  • Decreased mean plasma clearance of theophylline

Contraindications

Known hypersensitivity to Fluconazole or related triazole compounds

Pregnancy & Lactation

Not recommended for use in pregnancy or in women of childbearing potential unless adequate contraception is employed.

Precautions & Warnings

  • Abnormalities of hepatic, renal, hematological and other biochemical function tests observed in some patients
  • Rare cases of hepatic necrosis
  • Exfoliative cutaneous reactions in rare cases
  • Possible effect on endogenous steroid levels
  • Not recommended for use during lactation

Use in Special Populations

  • Elderly: Use normal dose if no evidence of renal impairment

Overdose Effects

Supportive measures and symptomatic treatment with gastric lavage if necessary may be adequate. Forced volume diuresis may increase the elimination rate.
