Indications

Pharmacology
- Triazole antifungal agent
- Inhibitor of fungal cytochrome P-450 dependent enzymes
- Potent inhibitor of fungal cell membrane synthesis of ergosterol
Dosage & Administration
- 150 mg single dose for Vaginal Candidiasis
- 200 mg in 1st day followed by 100 mg daily for 14 days for Oropharyngeal Candidiasis
- 200 mg in 1st day followed by 100 mg daily for 14-30 days for Oesophageal Candidiasis
- 150 mg weekly for 4-6 weeks for Tinea corporis/Tinea cruris/Tinea pedis/Other Tinea
- 50 mg daily for 20 days for Kerion
- 400 mg as a single dose for Pityriasis versicolor
- 150 mg weekly for 12 months for Onychomycosis
- Orally or by IV infusion, 400 mg on first day then 200-400 mg daily for Invasive candidal infections and cryptococcal infections (including meningitis)
- Orally or by IV infusion 200 mg daily for Prevention of cryptococcal meningitis
- Orally or by IV infusion, 50-400 mg daily for Prevention of fungal infections in immunocompromised patients
- 1-2 mg/kg daily for Superficial Candidiasis in children over 1 year
- 3-6 mg/kg daily for Systemic Candidiasis & Cryptococcal infection in children
Interaction
- Increases prothrombin time after warfarin administration
- Prolongs the serum half-life of concomitantly administered oral sulphonyl ureas
- Increases plasma concentrations of fuconazole when co-administered with hydrochlorothiazide
- Increases the level of phenytoin
- Results in a decrease in the AUC and shorter half-life when co-administered with rifampicin
- Affects the AUCs of ethinyl estradiol and levonorgestrel when combined with oral contraceptives
- Slowly increases cyclosporin concentrations
- No clinically significant impairment of fuconazole absorption when co-administered with food, cimetidine, antacids or following total body irradiation for bone marrow transplantation
- Results in a decrease in the mean plasma clearance of theophylline

- Known hypersensitivity to Fluconazole or related triazole compounds

Pregnancy & Lactation
- Adverse fetal effects seen in animals at high dose levels
- Should not be used in pregnancy or in women of childbearing potential unless adequate contraception is employed
Precautions & Warnings
- Abnormalities of hepatic, renal, haematological and other biochemical function tests observed in some patients
- Rare cases of hepatic necrosis
- Development of exfoliative cutaneous reactions
- Not recommended for use during lactation
- Unlikely to impair a patient's ability to drive or use machinery
Use in Special Populations
- Normal dose should be used in the elderly
- Dosage intervals or daily dosage should be adjusted in patients with renal impairment
- Excreted predominantly in the urine as unchanged drug
Overdose Effects
- Supportive measures and symptomatic treatment with gastric lavage if necessary
- Forced volume diuresis may increase the elimination rate
- Three-hour session of haemodialysis decreases plasma levels by approximately 50%
