Indications
- Schizophrenia

Pharmacology
- Atypical antipsychotic
- Partial agonist activity at Dopamine D2 receptors
- Partial agonist activity at Serotonin 5-HT 1A receptors
- Antagonist activity at Serotonin 5-HT 2A receptors
Dosage
- Adult schizophrenia initial dose: 10-15 mg/day
- Adolescent schizophrenia initial dose: 2 mg/day
- Bipolar mania adult monotherapy initial dose: 15 mg/day
- Bipolar mania pediatric monotherapy initial dose: 2 mg/day
- MDD adult initial dose: 2-5 mg/day
- Irritability associated with autistic disorder pediatric initial dose: 2 mg/day
Administration
- Oral formulations administered once daily without regard to meals
- IM injection administered with at least 2 hours between doses
Interaction
- Caution with other centrally acting drugs and alcohol
- Carbamazepine may increase clearance and lower blood levels
- Ketoconazole, quinidine, fluoxetine, or paroxetine may inhibit elimination and increase blood levels

Pregnancy & Lactation
Pregnancy category C. Avoid during pregnancy. Not recommended for breastfeeding women.
Precautions & Warnings
- Increased risk of cerebrovascular adverse events in elderly patients with dementia-related psychosis
- Increased risk of suicidality when used with antidepressants
- Discontinue if clinically appropriate for tardive dyskinesia
- Associated with metabolic changes, hyperglycemia, dyslipidemia, and body weight gain
- Use with caution in patients with known cardiovascular or cerebrovascular disease for orthostatic hypotension
- Use cautiously in patients with a history of seizures or convulsions
- Caution when operating machinery
Overdose Effects
No fatalities observed in doses up to 1080 mg. Symptoms include nausea, vomiting, asthenia, diarrhea, and somnolence.
