Pharmacology
Fluconazole is a triazole antifungal agent, and a potent inhibitor of fungal cytochrome P-450 dependent enzymes responsible for the synthesis of ergosterol.
Dosage & Administration
- Adults:
- Different doses for various indications
- Different doses for renal impairment
- Doses for prevention of fungal infections in immunocompromised patients
- Specific doses for children
- Doses for elderly patients
- Child over 1 year dosage:
- In Superficial Candidiasis: 1-2 mg/kg daily
- In Systemic Candidiasis & Cryptococcal infection: 3-6 mg/kg daily
Interaction
- Interaction with warfarin
- Interaction with oral sulfonylureas
- Interaction with hydrochlorothiazide
- Interaction with phenytoin
- Interaction with rifampicin
- Interaction with combined oral contraceptives
- Interaction with cyclosporine
- Interaction with food, cimetidine, antacids, or following total body irradiation
- Interaction with theophylline
Contraindications
Fluconazole should not be used in patients with known hypersensitivity to Fluconazole or related triazole compounds.
Side Effects
- Gastrointestinal symptoms
- Rarely encountered rash
- Rare cases of anaphylaxis
Pregnancy & Lactation
Fluconazole should not be used in pregnancy or in women of childbearing potential unless adequate contraception is employed.
Precautions & Warnings
- Observation of hepatic, renal, haematological, and other biochemical function tests
- Rare occurrence of exfoliative cutaneous reactions
- Discontinuation if rash develops
- Use during lactation not recommended
- Unlikely to impair ability to drive or use machinery
Use in Special Populations
- Use in the elderly
- Adjustment in dosage intervals for renal impairment
Overdose Effects
In the event of overdosage, supportive measures and symptomatic treatment should be administered, and forced volume diuresis may be considered.
