Pharmacology
Fluconazole is a triazole antifungal agent that inhibits fungal cytochrome P-450 dependent enzymes responsible for the synthesis of ergosterol in the fungal cell membrane.
Dosage & Administration
- Dosage varies based on the specific indication and patient demographics including adults, children over 1 year, elderly patients, and renal impairment
Interaction
- Interactions with warfarin, sulphonyl ureas, hydrochlorothiazide, phenytoin, rifampicin, combined oral contraceptives, cyclosporin, theophylline, food, cimetidine, antacids, and total body irradiation have been observed
Contraindications
Fluconazole should not be used in patients with known hypersensitivity to fluconazole or related triazole compounds.
Side Effects
- Common side effects include symptoms associated with the gastrointestinal tract, such as nausea, abdominal discomfort, diarrhea, and flatulence. Rare adverse events include rash and anaphylaxis.
Pregnancy & Lactation
Fluconazole should not be used in pregnancy or in women of childbearing potential unless adequate contraception is employed.
Precautions & Warnings
- Abnormalities in hepatic, renal, and hematological function tests have been observed in some patients, particularly those with serious underlying diseases. Rare instances of severe cutaneous reactions have been reported. Use during lactation is not recommended.
Use in Special Populations
- Elderly patients should use the normal dose if there is no evidence of renal impairment. Renal impairment necessitates dosage interval modifications based on creatinine clearance.
Overdose Effects
In the event of overdosage, supportive measures and symptomatic treatment with gastric lavage if necessary may be adequate. Hemodialysis may be considered to increase elimination rate.
