Pharmacology

Potent inhibitor of fungal cytochrome P-450 dependent enzymes responsible for the synthesis of ergosterol

Dosage & Administration

  • Adults:
    • 150 mg as a single dose for Vaginal Candidiasis
    • 200 mg in 1st day followed by 100 mg daily for 14 days for Oropharyngeal Candidiasis
    • 200 mg in 1st day followed by 100 mg daily for 14-30 days for Oesophageal Candidiasis
    • 150 mg weekly for 4-6 weeks for Tinea infections
    • 50 mg daily for 20 days for Kerion
    • 400 mg as a single dose for Pityriasis versicolor
    • 150 mg weekly for 12 months for Onychomycosis
    • Orally or by IV infusion, 400 mg on first day then 200-400 mg daily for invasive infections
    • Orally or by IV infusion 200 mg daily for prevention of cryptococcal meningitis
    • Orally or by IV infusion, 50-400 mg daily for prevention in immunocompromised patients
  • Children:
    • 1-2 mg/kg daily for Superficial Candidiasis
    • 3-6 mg/kg daily for Systemic Candidiasis & Cryptococcal infection
    • Up to 12 mg/kg daily for serious life-threatening infections
  • Special Population:
    • Elderly patient: Normal dose if no evidence of renal impairment
    • Renal Impairment: Dosage intervals modification based on creatinine clearance

Interaction

  • Interacts with warfarin, sulphonyl ureas, hydrochlorothiazide, phenytoin, rifampicin, oral contraceptives, cyclosporin, theophylline
  • Administration with food, cemetidine, antacids may not impair absorption
  • May decrease mean plasma clearance of theophyline by 18%

Contraindications

Hypersensitivity to Fluconazole or related triazole compounds

  • Rarely rash and anaphylaxis reported

Pregnancy & Lactation

Not recommended for use in pregnancy or lactation unless adequate contraception is employed

Precautions & Warnings

  • Abnormalities of hepatic, renal, haematological and other biochemical function tests observed
  • Hepatic necrosis reported rarely
  • Rare development of exfoliative cutaneous reactions
  • Use in nursing mothers not recommended

Use in Special Populations

  • Elderly: Normal dose if no evidence of renal impairment
  • Renal Impairment: Dosage intervals or daily dosage modification based on creatinine clearance

Overdose Effects

Supportive measures and symptomatic treatment recommended. Forced volume diuresis may increase elimination rate
