Pharmacology

Fluconazole is a triazole antifungal agent. It inhibits fungal cytochrome P-450 dependent enzymes essential for the synthesis of ergosterol in fungal cell membranes.

Dosage & Administration

  • Adults:
    • Different dosages for various indications such as Vaginal Candidiasis, Oropharyngeal Candidiasis, Oesophageal Candidiasis, Tinea Corporis, Tinea Cruris, Tinea Pedis, Kerion, Pityriasis Versicolor, Onychomycosis, Invasive candidal infections, and cryptococcal infections
    • Special dosages for prevention in immunocompromised patients
  • Children:
    • Different dosages based on weight for various indications
  • Use in Specific Population:
    • Elderly Patients: Normal dose if no evidence of renal impairment
    • Renal Impairment: Dosage adjustments based on creatinine clearance

Interaction

  • Interactions with warfarin, sulphonyl ureas, hydrochlorothiazide, phenytoin, rifampicin, oral contraceptives, cyclosporin, cimetidine, antacids, theophylline, and other medications

Contraindications

Not to be used in patients with known hypersensitivity to Fluconazole or related triazole compounds

Side Effects

  • Common gastrointestinal symptoms
  • Rare occurrences of rash
  • Rare cases of anaphylaxis

Pregnancy & Lactation

Not recommended for use in pregnancy or in women of childbearing potential unless adequate contraception is employed

Precautions & Warnings

  • Observation of hepatic, renal, haematological, and other biochemical function tests in some patients
  • Rare occurrences of severe cutaneous reactions
  • Discontinuation if rash develops in superficial or systemic fungal infections

Use in Special Populations

  • Elderly: Use normal dose if no evidence of renal impairment
  • Renal Impairment: Dosage adjustments based on creatinine clearance

Overdose Effects

Supportive measures, symptomatic treatment, and possible gastric lavage if necessary. Haemodialysis may decrease plasma levels by approximately 50%
