Pharmacology
- Fluconazole is a triazole antifungal agent
- Potent inhibitor of fungal cytochrome P-450 dependent enzymes
- Responsible for the synthesis of ergosterol

Interaction
- Increased prothrombin time after warfarin administration
- Prolongation of serum half-life of concomitantly administered oral sulphonylureas
- Increased plasma concentrations of fluconazole with co-administration of hydrochlorothiazide
- Increase in the level of phenytoin and decrease in AUC and shorter half-life with rifampicin co-administration
- Interaction with combined oral contraceptives, cyclosporin, theophylline, and other medications
- Effect of food, cimetidine, antacids, and total body irradiation on fluconazole absorption
Contraindications
- Fluconazole should not be used in patients with known hypersensitivity to Fluconazole or related triazole compounds
Side Effects
- Nausea, abdominal discomfort, diarrhea, flatulence
- Rare occurrence of rash and anaphylaxis
Pregnancy & Lactation
- Adverse effects observed in animals at high dose levels
- Little use during human pregnancy
- Not recommended in pregnancy or in women of childbearing potential unless adequate contraception is employed
Precautions & Warnings
- Observation of abnormalities in hepatic, renal, hematological, and biochemical function tests
- Potential occurrence of hepatic necrosis, exfoliative cutaneous reactions
- Discontinuation of therapy in case of rashes and close monitoring in invasive/systemic fungal infections
- Not recommended during lactation
Use in Special Populations
- Normal dose usage in the elderly
- Dosage modification in patients with renal impairment
Overdose Effects
- Supportive and symptomatic treatment
- Possibility of forced volume diuresis for increased elimination rate
- Approximately 50% decrease in plasma levels after a three-hour session of haemodialysis
