Indications

Pharmacology
- Triazole antifungal agent
- Potent inhibitor of fungal cytochrome P-450 dependent enzymes
- Responsible for the synthesis of ergosterol
Dosage & Administration
- Different dosage for various indications
- Dosage adjustments for children, elderly, and patients with renal impairment
- Specific dosage intervals based on creatinine clearance
- Oral or IV administration
Interaction
- Increased prothrombin time with warfarin
- Prolonged half-life of oral sulphonylureas
- Increased plasma concentrations with hydrochlorothiazide
- Increased level of phenytoin
- Decrease in AUC and shorter half-life with rifampicin
- Elevated AUC of certain contraceptive hormones
- Slow increase in cyclosporin concentrations
- No impairment of absorption with food, cimetidine, antacids, or following total body irradiation
Contraindications
- Known hypersensitivity to Fluconazole or related triazole compounds

- Nausea
- Abdominal discomfort
- Diarrhoea
- Flatulence
- Rarely encountered rash
- Rare cases of anaphylaxis
Pregnancy & Lactation
- Adverse effects seen in animals at high dose levels
- Not recommended in pregnancy or in women of childbearing potential without adequate contraception
Precautions & Warnings
- Observation of hepatic, renal, haematological and biochemical function tests
- Assessment of risk-benefit ratio in patients with significant rise of liver enzymes
- Rare cases of exfoliative cutaneous reactions
- Discontinue therapy on rash development
- Not recommended during lactation
- Unlikely to impair ability to drive or use machinery
Use in Special Populations
- Normal dose for the elderly without renal impairment
- Dosage adjustments for patients with renal impairment
- Excretion predominantly in urine
Overdose Effects
- Supportive measures and symptomatic treatment
- Possibility of gastric lavage
- Enhanced elimination rate with forced volume diuresis
- Approximately 50% decrease in plasma levels with haemodialysis
