Description

  • Onicon is an antifungal medication used for the treatment of various fungal infections such as vaginal candidiasis, oropharyngeal candidiasis, esophageal candidiasis, tinea corporis, tinea cruris, tinea pedis, kerion, pityriasis versicolor, onychomycosis, and invasive candidal infections.

Pharmacology

  • Potent inhibitor of fungal cytochrome P-450 dependent enzymes
  • Responsible for the synthesis of ergosterol

Dosage Administration Adults

  • Vaginal Candidiasis: 150 mg as a single dose
  • Oropharyngeal Candidiasis: 200 mg in 1st day followed by 100 mg daily for 14 days
  • Oesophageal Candidiasis: 200 mg in 1st day followed by 100 mg daily for 14-30 days
  • Tinea corporis/Tinea cruris/Tinea pedis/Other Tinea: 150 mg weekly for 4-6 weeks
  • Kerion: 50 mg daily for 20 days
  • Pityriasis versicolor: 400 mg as a single dose
  • Onychomycosis: 150 mg weekly for 12 months
  • Invasive candidal infections and cryptococcal infections (including meningitis): Orally or by IV infusion, 400 mg on first day then 200-400 mg daily
  • Prevention of cryptococcal meningitis: Orally or by IV infusion 200 mg daily
  • Prevention of fungal infections in immunocompromised patients: Orally or by IV infusion, 50-400 mg daily

Dosage Administration Children

  • In Superficial Candidiasis: 1-2 mg/kg daily
  • In Systemic Candidiasis & Cryptococcal infection: 3-6 mg/kg daily

Specific Populations Dosage

  • Elderly patient: The normal dose should be used if there is no evidence of renal impairment
  • Renal Impairment: No adjustment in single dose therapy is required. In multiple dose therapy of patients with renal impairment, normal doses should be given on days 1 and 2 of treatment and thereafter the dosage intervals should be modified based on creatinine clearance

Interaction Warfarin

  • Fluconazole increased the prothrombin time after warfarin administration in healthy males

Interaction Sulfonylureas

  • Prolongation of the serum half-life of concomitantly administered oral sulfonylureas in healthy volunteers. Co-administration to diabetic patients is possible, but the possibility of a hypoglycemic episode should be borne in mind

Interaction Hydrochlorothiazide

  • Co-administration to healthy volunteers receiving fluconazole increased plasma concentrations of fluconazole by 40%

Interaction Phenytoin

  • May increase the level of phenytoin to a clinically significant degree

Interaction Rifampicin

  • Results in a decrease in the AUC and shorter half-life of fluconazole. An increase in the fluconazole dose should be considered when co-administered with rifampicin

Interaction Oral Contraceptives

  • Multiple dose use of fluconazole at specific doses is unlikely to have an effect on the efficacy of the combined oral contraceptives
  • No clinically significant effect on endogenous steroid levels

Interaction Cyclosporin

  • Slowly increases cyclosporin concentrations
  • Patients receiving cyclosporin should undergo plasma concentration monitoring when receiving fluconazole

Interaction Other Medications

  • Drug-drug interaction studies with other medications have not been conducted, but such interactions may occur

Contraindications

  • Known hypersensitivity to Fluconazole or related triazole compounds

Side Effects

  • Nausea
  • Abdominal discomfort
  • Diarrhoea
  • Flatulence
  • Rash (incidence less than 1%)
  • Anaphylaxis (rarely encountered)

Pregnancy Lactation

  • Should not be used in pregnancy or in women of childbearing potential unless adequate contraception is employed
  • Found in human breast milk at concentrations similar to plasma, hence its use in nursing mothers is not recommended

Precautions Warnings

  • Abnormalities of hepatic, renal, haematological, and other biochemical function tests have been observed in some patients, particularly those with serious underlying diseases such as AIDS and cancer
  • Patients with severe underlying diseases who received multiple dose fluconazole had post-mortem findings which included hepatic necrosis. Causal relationship with fluconazole cannot be excluded
  • Patients have rarely developed exfoliative cutaneous reactions during treatment with fluconazole
  • Use during lactation is not recommended
  • Unlikely to impair a patient's ability to drive or use machinery

Overdose Effects

  • In the event of overdosage, supportive measures and symptomatic treatment with gastric lavage if necessary may be adequate
  • Plasma levels can be decreased by approximately 50% with a three-hour session of haemodialysis
