Indications

Pharmacology
Triazole antifungal agent, potent inhibitor of fungal cytochrome P-450 dependent enzymes, responsible for the synthesis of ergosterol
Dosage & Administration
- Different dosage for various indications
- Adults and child over 1 year dosages specified
- Specific dosage recommendations for elderly, renal impairment, and children
Interaction
- Increases prothrombin time after warfarin administration
- Prolongs serum half-life of concomitantly administered oral sulphonylureas
- Increased plasma concentrations of fluconazole with multiple-dose hydrochlorothiazide
- May increase levels of phenytoin
- Co-administration with rifampicin results in decreased AUC and shorter half-life
- No clinically significant impairment of absorption when co-administered with food, cimetidine, antacids, or following total body irradiation for bone marrow transplantation
Contraindications
Hypersensitivity to Fluconazole or related triazole compounds

- Nausea

- Flatulence
- Rarely encountered adverse events like rash (Incidence less than 1%)
- Anaphylaxis reported in rare cases
Pregnancy & Lactation
Adverse fetal effects in animals at dose levels associated with maternal toxicity, not recommended during pregnancy or in women of childbearing potential unless adequate contraception is employed
Precautions & Warnings
- Abnormalities of hepatic, renal, haematological, and other biochemical function tests observed in some patients
- Rare cases of hepatic necrosis observed, assess risk-benefit ratio in patients with significant rise of liver enzymes
- Rare development of exfoliative cutaneous reactions, particularly in AIDS patients
- Discontinue therapy in the event of rash attributable to fluconazole
- Not recommended during lactation
- Unlikely to impair a patient's ability to drive or use machinery
Use in Special Populations
- Dosing adjustments for elderly and patients with renal impairment based on creatinine clearance
- Excreted predominantly in urine as unchanged drug
- Monitoring of cyclosporin plasma concentration recommended
- No clinically significant impairment of absorption when co-administered with food, cimetidine, antacids, or following total body irradiation for bone marrow transplantation
- Interaction with theophylline may cause a decrease in plasma clearance
Overdose Effects
Supportive measures and symptomatic treatment, forced volume diuresis may increase elimination rate, haemodialysis decreases plasma levels by approximately 50%
